Vroeger deed kinesitherapeut kloppen op borst (tappotage), terwijl P onderste
boven ligt, om zo de secreties los te kloppen (ook gebruik makend van
zwaartekracht). Voldoende literatuur aanwezig dat dit niet werkt.
- Doel? Efficiënte en optimale mucus evacuatie
- Hoe? Verkrijgen van optimale expiratoire flow en luchtsnelheid door
ademhalingsmanier en ademhalingsdiepte te veranderen.
- Waarom? Om de mucus te verplaatsen dankzij de shearing forces. Hoe
groter de luchtsnelheid hoe beter de shearing forces.
BASIC MECHANISM OF AD
De bedoeling in autogene drainage is om een homogeen en
synchroon/gelijkmatige expiratoire flow creëren met optimale shearing force
over een zo groot mogelijk oppervlakte van de luchtwegen en vooral in de
aangedane luchtwegen.
Spelen met volumes, juiste techniek aanleren om maximale seccreties
te doen bewegen oiv luchtsnelheid
➔ Focus ligt hier vooral op de luchtwegen waar er veel secreties zijn en dus
obstructie.
INADEMEN BIJ AD
Alle longdelen worden gelijkmatig en gelijktijdig gevuld bij gezonde P. Gezonde
longdelen gaan zich snel vullen met lucht (lucht zoekt makkelijkste weg), terwijl
ziek longdelen zich minder snel gaan vullen.
- Adem in via de neus en met voorkeur met
een zeer lage luchtsnelheid
o Lucht moet achter de mucus
komen, P gaat dus door de neus
ademen en door dit traag te doen
zal er evenveel lucht in de
obstructieve als in de gezonde long
terechtkomen.
o Probleem = kortademig worden:
normaalgezien 12x per minuut
ademen, hier 5-6x per minuut ademt
Oplossing: op het einde van de ademhaling doe je een pauze,
want deze pauze zorgt dat de lucht tijd krijgt om in die
obstructieve long door te vloeien en er dus voor te zorgen
dat de druk gelijk wordt. Dit zorgt ervoor dat de secreties
uiteindelijk gaan kunnen bewegen.
- Na het inademen doe je een pauze van 3 à 4 seconden met een open
glottis (klep boven luchtpijp tijdens inademen en uitademen glottis
volledig openhouden om goed je longen te vullen met goede luchtsnelheid!)
- Hetzelfde, maar met een gesloten glottis
