1.1 Atherosclerose
Wat is atherosclerose?
• Een proces dat begint met een beschadiging van het endotheel door
o arteriële hypertensie
o roken
o diabetes mellitus
o hoog LDL-cholesterol
o aanleg
o leeftijd/geslacht
Stappen in het vormen van atherosclerose.
1. Vetslierten (lymfocyten, endotheel)
a. Onder het endotheel dat nog intact is
b. begint al op jonge leeftijd (>30 jaar)
2. Atheroomplaat/plaque
a. plaque groeit, puilt uit in lumen, endotheel nog intact
b. krijgt een zachte, necrotische kern
3. Verwikkelde plaque
a. Plaqueruptuur - kernmateriaal komt vrij
b. Trombose, embolie met gedeeltelijke of volledige occlusie

Wat gebeurt in de cardiomyocyten (cel van het hart)?
Calcium gaat de cel in en leid tot contractie van de myofilamenten (= eiwitten in de spiervezels).
Vervolgens word het calcium weer uitgepompt en ontspand het myofilament. Dit gegeven kost
energie. Dit halen we uit ATP dat afkomstig is van een reactie van glucose en O2. Wanneer er dus
geen O2 aan de cardiomyocyten kan komen door ischemie treed er necrose op. Hierdoor stijgt de
lactaat. Dit kan het calcium verhogen.
Wanneer er meer calcium in de cellen zit kan de cellen stijver worden. Omdat de cel zich minder goed
kan ontspannen.
