Bij een ongeval reageren kinderen zowel fysisch, psychologisch als fysiologisch anders dan volwassenen.
- Slecht inschattingsvermogen voor gevaar
- Kinderen kunnen pijn moeilijk beschrijven en lokaliseren (en alle symptomen ontkennen)

2.1. De initiële voorbereiding
- 112 bellen
- Idee van gewicht om medicatie te berekenen en leeftijdsadequaat materiaal klaar te leggen

o 1 – 5 jaar ® gewicht (kg) = (2 × leeftijd in jaren) + 8

2.2. De primary survey of eerste aanpak
= behandelen van levensbedreigende letsels.
2.2.1. Stap 1: Stop catastrofale uitwendige bloedingen

2.2.2. Stap 2: Airway

aangetast.
Nagaan of een nek vrijgegeven mag worden:
- Normaal onderzoek ® geen pijn bij palpatie of spontaan, geen zwelling, geen hematoom zichtbaar
- Normaal neurologisch onderzoek
- RX
o SCIWORA = spinal cord injury without radiological abnormality ® normale RX sluit geen schade aan
het ruggenmerg uit ® CT-scan
- CT-scan
2.2.4. Stap 4: Breathing
- Arbeid ® tachypnoe, intrekkingen, …
- Doeltreffendheid arbeid ® thoraxexpansies?
- Effecten respiratoire insufficiëntie op andere organen
Kijk en luister:
- Asymmetrie
- Abnormale bewegingen
- Subcutaan emfyseem (kraakt zoals sneeuw)
- Trachea in het midden?
Geen levensbedreigende letsels? ® Non-rebreathing masker met 10 – 15 l/min zuurstof.

- Tensiepneumothorax

- Ernstig hoofdtrauma met nood aan langdurige, gecontroleerde beademing
- Verstoord ademmechanisme zoals bij fladderthorax
- Persisterende luchtwegobstructie
Korte aanrijtijd = balloneren.
2.2.5. Stap 5: Circulatie
Circulatoire tekenen ® hartfrequentie, pols, capillaire refill, perifere pulsaties
Effecten circulatoire insufficiëntie op andere organen ® huidskleur, huidtemperatuur, ademhaling, bewustzijn
® 2 toegangswegen (IV of botnaald)
Vochtbolus bij dreigende shock ® liefst bloed, in trauma niet mogelijk dus kristalloïd bolus ® 10 ml/kg vocht.
Ziekenhuis verwittigen ® chirurg/radioloog verwittigen en eventueel massaal transfusie protocol opstarten.
2.2.6. Stap 6: Disability
APVU score ® alert, responsive to pain, responsive to voice, unresponsive
U? ® bewusteloos (GCS onder 8 = intubatie)
2.2.7. Stap 7: Exposure
