Laterale epicondalgie
Een tenniselleboog is een pijnlijke aandoening die wordt veroorzaakt door overbelasting van de
“extensor” spieren in uw arm en onderarm, vooral op de plaats waar de pezen zich vasthechten
aan ronde botuitstulpingen (epicondylus) aan de buitenkant of het laterale deel van de elleboog. De
spieren die u gebruikt om iets te pakken, om te draaien en om voorwerpen met uw hand te dragen
hechten zich allemaal aan de “laterale epicondylus” bij de elleboog. Daarom doet een beweging
van de pols of de hand pijn aan de elleboog.

Ontstaansmechanisme: degeneratief door microtrauma, eerst degeneratieve angiogenese (de
vorming van nieuwe bloedvaten vanuit bestaande bloedvaten), dan fibrosis en calcificatie

Verschijnselen
Pijn rond laterale epicondyl (soms met uitstraling naar de onderarm) bij:
▪ Extensie van de pols (tegen weerstand) bij rechte elleboog
▪ Supinatie (tegen weerstand)
▪ Druk op of rond laterale epicondyl
▪ Knijpen

Flexie en extensie elleboog zijn vrijwel altijd pijnloos en volledig!

Prognostische factoren:
• langdurige klachten
• ernstige pijn
• dominante arm
• recidief
• nekklachten
• hoge lichamelijke en psychische belasting



Onderzoek
Mills test
Deze test is ontworpen om de extensor carpi radialis longus en brevis maximaal op te rekken en
daarmee te provoceren.
1. Fixeren humerus en palperen laterale epicondyl
2. Passief pronatie arm van de patiënt en flexie van de pols
3. Volledige extensie van de elleboog maken



De test is positief wanneer de patiënt spreekt over plotselinge pijn aan de laterale epicondyl tijdens
het uitvoeren van deze handeling.
