1 Environmental Health and the Burden of Disease
09/11/2023

This module introduces the concepts of health, public health and environmental
health. In addition, it provides an overview of the global burden of disease that
is attributable to pollution.

1.1 Context

1.1.1 What is health?
• State of complete physical, mental and social well-being
• There are a lot of factors that influence health
o Work env., education, age, sex, housing, …

1

,1.1.2 What is public Health?
• Preventing diseases
• Prolonging life
• Promoting health

1. Surveillance:
o Collect information on the status of disease
▪ Where, who, when?
o => establish health objectives and priorities
2. Health services
3. Disease prevention
o Primary prevention: prevent getting the disease
o Secondary prevention: screening and early diagnosis
o Tertiary prevention: ziekte vertragen
4. Health promotion
o Empowering people to life more healthy
5. Health protection
o Protect the population against hazards
o Bv: luchtkwaliteit, gezond drinkwater

1.1.3 What is Environmental Health?
• All factors (physical, chemical, biological) external to the human body
which may affect health
• Studies between exposure and development

4 disciplines in env. Health

1. Environmental toxicology:
o Studies in animal models or cells
2. Environmental epidemiology:
o Studies in populations
3. Health protection:
o Minimizing the exposure to health hazards
o By ensuring safety
4. Policy:
o Legislation to protect the population

,Samenvatting Environmental Health

1.2 Historical case studies

1.2.1 The Meuse Valley fog (12/1930)
• Zeer dichte mist over heel België
• In de Meuse Vallei:
o 10 keer meer doden dan gewoonlijk
o Ademhalingssymptomen
• What was the cause of symptoms and death?
o Industrie stoot gassen, roken en stof uit
o SO2: concentratie was hoog genoeg om symptomen en doden te
verklaren
• Why the Meuse Valley?
o => Meteorologische omstandigheden:
▪ Anticycloon boven de vallei (er ontstaat inversie, vervuiling
kan niet weg)
▪ Zeer zwakke wind (geen verdunning)
▪ Temperatuur inversie boven de schoorstenen in de vallei

1.2.2 The London fog (12/1952)
• Piek in luchtvervuiling
o Acuut effect: 4 000 extra doden tijdens de week van mist
o Lange termijn effect: 8 000 extra doden in totaal
• Mortality increases with SO2 + long term effect
o SO2 conc. Terug naar normaal, maar aantal doden blijft hoog

1.2.3 Investigation of 12 asthma outbreaks in Barcelona (1981 – 1986)
• Dichtbij de haven
• Uitladen van sojabonen

1.2.4 Conclusie
• Outbreaks: not only about infectious diseases
• Meteorology and air pollution
• Short-term effects vs long-term effects

3

, 1.3 Environmental pollution and the burden of disease

1.3.1 Pollution
“…unwanted, often dangerous, material that is introduced into the Earth’s
environment as the result of human activity, that threatens human health,
and that harms ecosystems.”

• 9 million deaths due to pollution every year ≈ aantal doden door roken
o Meeste doden door luchtvervuiling
o Meeste doden in Afrika en Azië
o Meeste doden in ‘lower and upper middle country income’
▪ => Vooruitgang veroorzaakt veel vervuiling

1.3.2 DALY’s (Disability-adjusted life-year)
• Number of years lost due to ill-health, disability or early death
• Young children are most vulnerable to DALY

1.3.3 Premature deaths
• Elderly are most vulnerable
• Very young children are not so resilient
• Mensen die vroeger sterven dan de verwachting

1.4 Current topics in environmental Health

1.4.1 Radiofrequency
• Ionizing radiation
• Non-ionizing radiation
o Daily use
• Over exposure => damage
o DNA mutations
o Class 2B: Possibly Carcinogenic to humans

1.4.2 Water pollution
• Microbes
• Chemicals
o Chemicals in drinking water
▪ Natural geochemicals
▪ Water treatment
▪ Human activities
▪ Packaging
o Recreational bathing
▪ Chlorine + organic matter => disinfection by-products (DBP)
▪ Can be carcinogenic
• Unsafe water, sanitation and handwashing = 11th cause of death
