Waterbalans:
-> in orde indien even veel inname als verlies

de plasmaosmolaliteit stijgt.

- de nieren gaan maximaal dilueren
- bij een normaal dieet: 600mOsm excretie via urine per dag
- volume urine = mOsm per dag / osmolaliteit in urine
- alles wat je hebt binnenhouden = max conc. 600mOsm/1200mOsm = 0,5l
alles wat je hebt uitplassen = max dil. 600mOsm/50mOsm = 12l
-> conc./dil. Afhankelijk van:
- ADH
- creëren hyperosmolair medullair interstitium (ruimte tss bloedbaan en cellen)
-> gecreëerd door countercurrent multiplicatie in de Lis van Henle
-> ureum recycling in renale medulla
- behouden door: vasa recta (countercurrent exchange) = bloedvoorziening afkomstig van
efferente artriole.
-> countercurrent multiplicatie:
- hairpin van lis van henle, Bevat verschillende delen met verschillende eigenschappen.
- links doorlaatbaar voor water
- rechts doorlaatbaar voor zouten
- de NaCl pompen zorgen voor actieve verplaatsing
- in het water doorlatend deel
