1 Inleiding
Stoornissen van respiratoire functies:
- Bemoeilijking van beweging van lucht in of uit de alveolen
- Hinder bloedflow doorheen longcapillairen
- Inadequate gasuitwisseling tussen alveolen en capillairen
Longen hebben reservecapaciteit stadia in longaandoeningen: asymptomatisch – manifeste
decompensatietoestand
- Tekortschieten ademhaling leidt tot hypoxemie en evt tot hypercapnie
- Uitwisseling van CO2 thv alveolen gebeurt sneller dan uitwisseling van O 2
o Geïsoleerde hypercapnie = zeldzaam
- Respiratoire insufficiëntie:
o PaO2 < 80 mmHg
o PaCO2 > 45 mmHg
2 Pathofysiologie van hypoxie en hypercapnie
2.1 Hypoxie = anoxie
2.1.1 Oorzaken
2.1.1.1 Respiratoire oorzaken
Ventilatie-perfusie (VA / QA) onevenwicht :
- Longzones met hoge ratio : toename dode-ruimte-ventilatie
- Longzones met lage ratio: functionele rechts-links shunt
- Bij vele pathologieën zijn er zones met hoge én lage ratio
o Normale PaCO2
o Verlaagde PaO2 want longgebieden met hoge ventilatie kunnen het verlies van O 2–
overdracht in gebieden met lage ventilatie niet compenseren (vorm van O 2-
dissociatiecurve)
, Shunt:
- Extreme vorm van ventilatie-perfusie-onevenwicht:
o Continu niet-geoxygeneerd bloed toegevoegd aan bloed dat in longen wel van O 2
werd voorzien
Door stoornis in pulmonaire circulatie
Globale hypoventilatie:
- Door talrijke oorzaken
- Steeds hypercapnie
- Dmv alveolaire gasvergelijking hypoxie voorspellen:
Diffusiestoornissen (alveolocapillair block)
- Vooral hypoxie bij inspanning
o Leidt in veel mindere mate tot hypoxie dan men vroeger dacht
COPD
Bloedvatpathologie bv arteriële hypertensie
Interstitiële pathologie bv oedeem en sarcoïdose
2.1.1.2 Niet-respiratoire oorzaken
Lage PO2 in de ingeademde lucht (hypoxische hypoxie)
Te trage of onvoldoende bloedsomloop (stagnatie hypoxie)
Te weinig Hb (anemische hypoxie) of verminderde bindingscapaciteit van Hb (CO-intoxicatie,
methemoglobine, sulfhemoglobine)
Cellulaire stoornissen (histotoxische hypoxie)
2.1.2 Symptomen en begeleidende verschijnselen
2.1.2.1 Weerslag op de ademhaling
Dyspnee & tachypnee
2.1.2.2 Weerslag op het bloed
Polyglobulie (polycythemie):
- Erythropoïetine-vorming RBC-productie
o Verhoging O2-dragende capaciteit
o Vermeerdering bloedviscositeit risico op trombose
Cyanose:
- = blauwpaarse verkleuring huid & slijmvliezen
- > 5g gereduceerd Hb aanwezig per 100 ml bloed
o Gemakkelijker aanwezig bij polyglobulie dan anemie
- Perifere cyanose
o Bij stagnatie bv door koude
- Centrale cyanose
o Bv bij respiratoire aandoeningen
o Ook tong en mondslijmvlies cyanotisch
