, INLEIDING
In 2020 overleden 7.130 personen aan de gevolgen van hartfalen en in 2019
bedroegen de geraamde zorguitgaven 527 miljoen voor hartfalen. Zonder
effectieve maatregelen is de verwachting dat het aantal mensen met hartfalen in
de periode 2018-2040 naar verwachting met 72% zal stijgen (Volksgezondheid
en Zorg, 2022).
Onderstaand verslag betreft het tentamen ‘Individuele preventie” van Module 6
‘Samenwerken aan gezondheidsbevordering’. In deze module staan de Canadian
Medical Education Directions for Specialist (CanMEDS) rollen:
‘gezondheidsbevorderaar’, 'reflectieve EBP professional', ‘samenwerkingspartner’
en ‘communicator’ centraal (Lambregts et al., 2020).
De uitgewerkte casus betreft een zorgvrager vanuit de verpleegkundige wijkzorg
TSN in Roden, met het gezondheidsprobleem hartfalen. Door middel van
gezamenlijke besluitvorming is samen met de zorgvrager een plan voor de
bevordering van een gezonde leefstijl opgesteld. Tevens wordt het gezondheid
en leefstijlprobleem op verschillende wijzen gesignaleerd en geanalyseerd.
