,Voorwoord
Wij als eerstejaars studenten aan de Fontys Hogeschool te Eindhoven hebben een
verpleegplan opgesteld aan de hand van de casus van Jaap. De Kern b lessen
hebben ons geholpen met het opstellen hiervan. Jaap is een jongen van 18 jaar met
het syndroom van down en een matig verstandelijke beperking. Aan de hand van de
casus hebben wij ons verdiept in de verschillende onderwerpen die aan bod moesten
komen in het verpleegplan. Zo hebben we een anamnese gedaan volgens de
gezondheidspatronen van Gordon, ook hebben we vijf diagnoses opgesteld waarvan
we de vier belangrijkste hebben uitgewerkt. Per diagnose hebben we twee doelen
opgesteld met daarbij passende interventies en uiteindelijk hebben we dit nog
geëvalueerd.
