Prevalente ongeveer 740.000 mensen met de diagnose Diabetes Mellitus in Nederland.

Incidente jaarlijks komen daar 72.500 nieuwe gevallen bij.

De prevalente van DM II is het hoogst onder Hindoestanen.

DM II is een chronisch progressieve stofwisselingsziekte waarbij de glucoseregulate ontregeld is door
verhoogde insulineresistente ennof het onvoldoende aanwezig zijn van insuline. Overgewicht en
onvoldoende lichaamsbeweging spelen, naast genetsche aanleg, een belangrijke rol in het
tekortschieten van de insulineproducte en ontstaan van DM II.

Doelstelling ketenzorgprogramma het multdisciplinair behandelen van patinten met Diabetes
Mellitus type II. Hierbij staat kwaliteit van leven centraal en wordt er gestreefd naar optmale
ondersteuning van zelfmanagement.

De doelen zijn:
- Verbeteren kwaliteit van leven.
- Afname morbiditeit en mortaliteit (verkleinen kans op korte- en lange termijn complicatess.
Uniforme en adequate aanpak (met professionele standaarden en regionale procedureafsprakens.
Ondersteunen en versterken van zelfmanagement.
Zorg waar mogelijk verschuiven naar de eerste lijn.
