Dit plan van aanpak is geschreven in het kader van de U2 stage in de thuiszorgorganisatie
ICARE te Emmermeer. Gedurende de stage wordt door de student de vijf verpleegkundige
beroepsrollen geïntegreerd in een project, dat uitgevoerd wordt binnen het komende half
jaar. Het plan van aanpak is opgebouwd uit verschillende hoofdstukken. In hoofdstuk 1 wordt
het projectvoorstel beschreven, hoofdstuk 2 bestaat uit de verantwoording van de
leeruitkomsten en in hoofdstuk 3 zullen de persoonlijke leerdoelen van de student
beschreven worden.
