Inleiding
Er bestaat een groot spectrum van
fungale ziekten:
Ziektelast van fungale infecties
- Oppervlakkige infecties van huid, haar en nagels:
o Zijn de frequentst voorkomende fungale infecties bij de mens
o 1/5 volwassenen hebben last van tinea pedis of ‘Athlete’s foot’
- Mucosale infecties:
o Spruw (orale candidiasis) komt voor bij:
+/- 2,5 miljoen mensen die leven met HIV/AIDS en die geen
antiretrovirale therapie nemen
> 1 miljoen andere patiënten
o Candida vulvovaginitis:
komt bij 50-75% van de vrouwen minstens één keer voor
gedurende hun leven
5-8% hebben last van minstens 4 episodes per jaar
- Invasieve fungale infecties
o Veel lagere incidentie, maar enorm hoge mortaliteit
o De incidentie neemt toe door een toename aan patiënten die risico
lopen voor deze infecties (Astma, cystic-fibrose,
transplantatiepatiënten, …)
o Ongeveer 2,5 miljoen mensen sterven jaarlijks wereldwijd omwille
van deze infecties
o De meeste doden zijn te wijten aan infecties door Cryptococcus,
Candida, Aspergillus en Pneumocystis
Fungi zijn eukaryoten!
- Lijken dus meer op de gastheer (de mens) dan
bacteriën -> fungale infecties zijn moeilijker
te behandelen dan bacteriële infecties

- Gisten
o Eencellig, bij kweek vormen deze glanzende kolonies gelijkaardig
aan bacteriën
- Filamenteuze fungi
o Zie je op oud brood of bij vochtproblemen in badkamer, …
o Meercellig, pluizige vorm
o 1 schimmeldraad = Hyphe
Geheel van hyphen is een mycelium
Aan de top van de draden gebeurt de verlenging
- Soms mengvormen qua morfologie
Samenstelling celwand:
- Fosfolipiden dubbelmembraan
o Bevat Ergosterol
= tegenhanger van het humane cholesterol. Veel antifungale
middelen werken in op Ergosterol
o Hierbovenop typisch chitine, suikerketen en mananen.
Cryptococcen hebben daarbovenop nog eens een kapsel
