ECMO
Inleiding
ECMO als mechanische ondersteuning, vnl in grote of universitaire ziekenhuizen.
Bij ernstig hart en/of longfalen tot deze:
Hersteld zijn om zelf terug te werken
Tot er evt een alternatieve therapie kan opgestart worden (bv. transplantatie)
Om tijd te winnen tot er een diagnose kan gesteld worden
ECMO =extracorporele membraanoxygenatie
Oorspronkelijk: bloed voorzien van O2, later: CO2 verwijderen = extra corporele koolstofdioxideverwijdering.
Aanvankelijk: postoperatieve ondersteuning bij pt na hartchirurgie, laatste 20 jaar: levens- en
orgaanondersteunende maatregelen.
Nieuwe term: ECLS: extracorporele levensondersteuning.
Tegenwoordig is ECMO eigen aan IZ en niet alleen meer perioperatief. Continue monitoring van specifieke
ECMO-apparatuur: overgenomen door speciaal opgeleide verpleegkundige: perfusionist.
Belang heparine?
= Heparine voorkomt dat het bloed gaat stollen. Bij het toedienen van te weinig heparine kunnen zich kleine
bloedstolsels in de bloedbaan vormen, wat tot beschadigingen aan vitale organen kan leiden. Een bloeding of
bloedstolsel in de hersenen kan bijvoorbeeld leiden tot een beroerte. Ook kunnen er infecties optreden.
Wat is ECMO?
= ExtraCorporele MembraanOxygenatie
= Buiten het lichaam toedienen van zuurstof over een membraan.
= Veneuze bloed via kunstmatige weg buiten het lichaam gebracht om via een kunstmatig membraan
geoxygeneerd te worden. Zuurstofrijk bloed wordt teruggegeven aan de pt pulmonale en/of cardiale
ondersteuning aan pt met ernstig hart- en/of longfalen.
2 soorten ECMO: VV-ECMO en VA-ECMO
1. VV-ECMO
= Veno-Veneuze ECMO
= Gebruikt bij reeds beademde pt met ernstige hypoxische (tekort O2 in bloed) of hypercapnische (teveel CO2
in bloed) problemen Bloedgassen interpreteren: pH, CO2 en dan HCO3.
Veel toegepast tijdens COVID periode
= Fungeert als een soort kunstlong: veel minder ingezet op cardiale ondersteuning.
Indicatie:
= vnl toegepast bij pt met ARDS, hypoxische respiratoire insufficiëntie, of respiratoire achteruitgang in
afwachting tot longtransplantatie.
ARDS Hypoxische respiratoire insufficiëntie
= Acute Respiratory Distress Syndrome = uitgesproken O2 tekort vnl veroorzaakt door een
= longen falen in hun functie ventilatie/perfusie (VQ) mismatch.
= kan bevestigd worden door verhouding PaO2 en FiO2 te bekijken.
Normaal: ongeveer 452
- Milde ARDS: 200-300
- Matige ARDS: <200
- Ernstige ARDS: <100
Bepalen door bloedgasanalyse:
Bv. beademde covidpt met FiO2 90%, PaO2 60 mmHg:
PF-ratio: 60/0,9=66,67 Dus hoge FiO2 en lage PaO2 = lage SaO2
