Encéphalopathie hépatique
Définition
L’encéphalopathie hépatique est une manifestation neuropsychiatrique de l’atteinte
hépatique considérée comme une complication terminale de la maladie du foie.
Physiopathologie
L'encéphalopathie hépatique peut se produire dans tout trouble hépatique où une
lésion permet à l'ammoniaque de rejoindre la circulation systémique sans avoir été
préalablement détoxifiée par le foie. L'ammoniaque qui est obtenue à la suite de la
désamination enzymatique et bactérienne des acides aminés dans l'intestin est
acheminée vers le foie par la circulation portale où elle est convertie en urée avant
d'être excrétée par les reins. Lorsque le foie est incapable de convertir l'ammoniaque
en urée, les concentrations de cette substances dans la circulation systémique
augmentent. L'ammoniaque franchit donc la barrière hématoencéphalique et
provoque des manifestations neurologiques toxiques.
Étiologie
Les différentes causes comprennent les effets neurotoxiques de l’ammoniaque, une
neurotransmission anormale, un oedème des astrocytes et les cytokines
inflammatoires.
