board of trustees
2) board of trustees must be composed of 1 rep from each participating facility
3) each member must deposit greater of 2x monthly average or $5k in an acct maintained by board of
trustees in the name of the self-insurance pool in a chartered commercial bank in FL
4) compensates any resident suffering financial loss as the result of misconduct by on or more of the
member licensees


If no surety bond or self-insurance pool
if resident requests facility hold funds, facility must notify AHCA, in writing, of the request and apply
for a surety bond or participation in self-insurance pool within 7 business days

- must have policies and procedures governing all services provided in the facility
- P&P must be reviewed at least annually
- must be reviewed by administrator, medical director, and DON
