immuunziekten te onderdrukken

II. Immunosuppressiva bij transplantatie
1. Glucocorticoïden

Remmen immuunrespons via complexe mechanismen
o O.a. inhibitie van genen die coderen voor IL-2 en andere cytokines zoals TNF-
en interferon- (IFNIFN)
2. Calcineurine-inhibitoren
Werking van calcineurine
o Bij activatie van T-cellen tijdens immuunrespons, stijgt intracellulaire
calciumconcentratie
o Dit leidt samen met calmodulin tot activatie van calcineurine B
o Calcineurine B activeert calcineurine A
o Calcineurine A (IFNfosfatase) defosforyleert de nuclear factor of activated T-cells
(IFNNFAT)
Deze factor vóór defosforylatie: in inactieve toestand in cytosol
Na defosforylatie: migreert naar kern en leidt daar tot transcriptie van
IL-2 en andere cytokines
Ciclosporine bindt in T-lymfocyten met het immunophiline cyclophiline
o Dit complex bindt met calcineurine, waardoor dit geïnactiveerd wordt
o Dit verhindert defosforylatie van NFAT en dus de activatie ervan

, Tacrolimus (IFNFK506) heeft gelijkaardig werkingsmechanisme, maar het betrokken
immunophiline verschilt van dit van cyclosporine (IFNhier: FKBP-12)
3. Antiproliferatieve en antimetabole agentia
3.1. Azathioprine
Cytostatica die interfereren met celdeling hebben een immunosuppressief effect
omdat zij de klonale proliferatie van T-en B-lymfocyten inhiberen
Toxiciteit is vaak te uitgesproken om ze langdurig als immunosuppressiva te
gebruiken
Wordt frequent als immunosuppressivum gebruikt
Azathioprine = imidazolederivaat van 6-mercaptopurine
o Wordt in vivo gesplitst tot 6-mercaptopurine
o 6-mercaptopurine wordt verder omgezet tot additionele metabolieten
o Metabolieten remmen de de novo synthese van purines
Blijkt effectiever te zijn als immunosuppressivum dan 6-mercaptopurine zelf
o Waarschijnlijk door verschillen in opname in targetcellen of
farmacokinetische verschillen in bekomen metabolieten
3.2. Mycofenozuur
Selectieve inhibitor van inosinemonofosfaatdehydrogenase
o Enzym dat essentieel is voor synthese van het purine guanine
B-en T-lymfocyten zeer sterk afhankelijk van deze pathway voor hun proliferatie

o Gevolg: mycofenolzuur remt selectief de proliferatie van B-en T-lymfocyten
