Gluten bestaan uit prolamines (niet wateroplosbaar)

- Gluteninen
CLASSIFICATIE VAN GLUTENGERELATEERDE AANDOENINGEN
Coeliakie is geen allergie maar een auto-immuunziekte
- Gluten komen exogeen binnen dus normaal wel allergie -> coeliakie is uitzondering
- De reactie is niet op de gluten gericht maar op de darmen, gluten zijn de trigger

KLINISCH BEELD
- Klassieke malabsorptie is zeldzaam en ziekte wordt vaak gediagnosticeerd obv milde, GI-
symptomen of geassocieerde aandoeingen
Klinische verschijnselen
- Zeer heterogeen, veel verschillende presentatievormen
- Vaak presentatie tijdens kindertijd of 50-60 jaar oud
- Symptomen
o Chronische diarree (malabsorptie -> darmwand ontsteekt -> geen
absorptie -> hogere osm waarde -> osmotsiche diarree)
o Anorexie
o Braken
o Humeurigheid (omdat immuunsysteem heel hard moet werken kost het
veel energie en wordt je moe)
o Algemene malaise
o Onverklaarde ferriprieve anemie (malabsorptie ijzer)
o Huiduitslag
- Kan ook huidziekte (dermatitis herpetiformis), waarbij vnl thv de
ellebogen, knieën en billen jeukende huiduitslag met blaasjes
verschijnt
Klinisch beeld
- Inflammatie -> villusatrofie met crypthyperplasie
o Ernstiger proximaal dan distaal (duodenum)
o Proximaal duodenum ligt net na de maag, waarom daar meer symptomen -> de gluten
komen daar het minst verteerd toe, dus de meeste reactie mogelijk
- Verdwijnt bij aangepast dieet

, Normaal Coeliakie
PATHOFYSIOLOGIE
1. Gliadine wordt doorheen enterocyt geabsorbeerd -> komt in interstitiële ruimte terecht
2. Gliadine bindt op het enzym weefsel (tissue) transglutaminase (tTG). tTG en gliadine vormen
tijdelijk een sterke covalente binding
3. APC (macrofagen) hebben door het HLA-DQ2(8) gen , HLA-DQ2(8) receptoren
4. Het tTG-gliadine complex wordt herkend door deze HLA-DQ2(8) receptoren op het opp van
de APC
5. Het tTG-gliadinecomplex, gebonden op de HLA-receptoren, activeert T-cellen
6. T-cellen: maken ontstekingsmediatoren IFN-gamma aan => zorgt voor ontsteking
(villusatrofie)
7. De T-cellen activeren op hun beurt B-cellen
8. B-cellen: maken 3 antilichamen aan: antigliadine, anti-endomysium (EMA) en anti-tTG
antilichamen (IgA)
