De afgelopen 8 weken hebben wij een verpleegplan in elkaar gezet aan de hand van de casus van
Emine.
Allereerst hebben we de anamnese afgenomen met behulp van de 11 gezondheidspatronen van
Gordon. Daarbij hebben we informatie uit de casus gehaald en vragen gesteld aan de patiënt, bij
meet the patiënt. Vervolgens hebben we uit de verschillende problemen diagnoses opgesteld met
behulp van de PES-structuur. Daarbij hebben we passende doelen opgesteld met daarbij horende
interventies. Tot slot hebben we aan de hand van de interventies evaluatie vragen opgesteld.
Dit verpleegplan draagt bij aan het verlenen van zorg op maat voor Emine.
