1.4.3. Graadbepaling
Graad 0
o Een volledige intacte huid
Graad 1
o Ontstaan van een oppervlakkig huidletsel. Het letsel beperkt
zicht tot de epidermis en de dermis, de wonde heeft een
beperkte neiging tot bloeden, er is geen geurprobleem, het kan
reeds pijnlijk zijn en soms is er aanwezigheid van jeuk
Graad 2
o Aantasting van het onderhuids weefsel. Er is een toename van het
wondvocht. Dit zal in matige hoeveelheid afgescheiden worden. De
neiging tot bloeden neemt toe en het ulcus is meestal pijnlijk. Ook
de jeuk is meer aanwezig.
Graad 3
o Aantasting van het subcutaan vetweefsel, met eventuele
aantasting van de dieper gelegen structuren. Er is een grotere
toename van het exsudaat de wonde is meestal pijnlijk en jeuk.
Ook de neiging tot bloeden is toegenomen
