Fysiologische regulatie van

o geregeld door ACTH geregeld door CRH, en oefent op zijn

Symtomen: diverse effecten van plotse vrijgave catecholamine: 6 P’s
• Pressure: hypertensie maar ook hypotensie
• Pain: hoofdpijn (occipitaal)
• Palpitaties (hartkloppingen)
• Perspiration (zweten) - angst
• Palor (bleekheid) - nadien flush
• Paroxysmaal: spontaan of uitgelokt (contrast, druk, fysische inspanning)
Diagnose:

Behandeling: heelkunde (meestal laparascopie)
• cave: goede preoperatieve voorbereiding essentieel: eerst a-blocker enkele weken op voorhand,
nadien zo nodig ß-blockers - CCA
2 Cortex
2.1 Hypofunctie = bijnierschorsinsufficiëntie
Oorzaken:
• Primair: destructie bijnierschors = ziekte van Addison
o auto-immuuun (al dan niet geassocieerd)
o tumoren
o infectie
o vasculair
o iatrogeen
o congenital (enzyme stoornissen)
• Secundair: door uitval hypothalamus, hypofyse as
• Tertiair: iatrogeen, na plotse stop na langdurig GCC MEEST FREQUENT
Symptomen:
• Doodmoe - spierzwakte - acute bijniercrisis
• GI: misselijk (cave: medicatie niet meer innemen)
• Hypotensie vooral orthostatisch
• Zouthonger, hypoglycemie
• Seksueel: impotentie, amenorhee, verminderde beharing
• Hyperpigmentatie (bruin) enkel bij primaire vorm (verhoogd ACTH stimuleert melatonine) -
vitiligo
Diagnose:
• Cortisol deficiëntie
o Cortisol gedaald
§ hetzij basaal ‘s morgens
§ hetzij na stimulatie ACTH (primaire vorm) of na insulinetolerantietest (ITT)
(secundaire vorm)
o ACTH gestegen als primaire vorm
• Aldosteron deficientie
o hyponatremie, hyperkaliemie (hypercalcemie)
o aldosterone gedaald
o plasma renine activitait gestegen
o verder: glycemie, HCO3 laag, calcium, TSH hoog (want meer kans op Hashimoto, DM1)
Behandeling:
• cortisol deficiëntie: hydrocortison PO (IV)
• aldosteron deficientie: 9α-fludrocortison PO
• identificatiekaart en educatie (stress rules)
