OZT7aLes 1 anatomie: Uterus, tubae en ovariae
Leerdoel 1: Uitleggen wat we bij thema vrouwelijke geslachtsorganen in leerjaar 2 hebben
behandeld;
Leerdoel 2: De bouw en ligging van uterus, tubae en ovariae beschrijven;
Embryologie:
• Gangen van Wolff (blauw): vorming baarmoeder + tubae
• Gangen van Muller (rood): vorming vascularisatie en ovariae
→ Gang van Muller komen via beide zijde bij elkaar en vormen zo de uterus en tubae
Uterus: ligt tussen blaas en rectum in
• Peritoneum: gedeeltelijk intraperitoneaal.
Cervix ligt subperitoneaal
• Fornices: plooi die bestaat door dat portio
uitsteekt in vagina
• Cavum Douglas/ Exacavatio recto uterine:
ruimte tussen uterus en rectum
→ Resten van ontstekingsprocessen hopen
hierin op. Dit is de diepste holte in buikholte
• Exacavatio vesica uterina: ruimte tussen
uterus en blaas
• Ligging: versio (schuin omhoog) en flexio
(buiging) en positio (blijft op deze postie
hangen)
• Stand uterus:
– Retroflexio: knik in uterus
– Retroversio: knik in begin van uterus
(baarmoedermond)
– Retroversie en retroflexie
– Positio
• Bouw uterus:
– Fundus uteri: bovenste gedeelte
– Corpus uteri:
– Isthmus uteri: waar smaller wordt onderaan in
uterus
– Cervix uteri: baarmoedermond
– Canalis cervicalis:
– Ostium uteri externum: opening uterus
– Portio vaginalis: gedeelte dat in vagina uitsteekt
– Cavitas uteri: baarmoederholte
– Fornix anterior, posterior, lateralis: plooien tussen
portio en vaginawand
• Wand:
– Endometrium: tunica mucosa
– Myometrium: tunica muscularis
– Perimetrium: tunica serosa
Tubae:
• Bouw:
, – Infundibulum: bredere structuren in de fimbriae
– Ampulla: bredere gedeelte
– Isthmus tubae uterina: smallere gedeelte
– Pars uterina: gedeelte in de baarmoederwand
– Fimbriae: draadjes voor opzuigen van eitjes
– Ostium abdominale: opening aan de tuba/buikholte
– Ostium uterinum: opening naar de uterus
– Bij een EUG (extra Uterine Graviditeit) vindt de eicel zich meestal plaats in de tuba uterina,
pars uterina → versmalling
• Wand
– Tunica mucosa: Plooien
– Tunica muscularis: meerdere dunne lagen
– Tunica serosa: peritoneale afdekking
Ovariae:
Leerdoel 3: De banden en peritoneale structuren bij de inwendige vrouwelijke geslachtsorganen
benoemen en herkennen in een figuur;
Peritoneum ligt over bovenzijde uterus heen
Ligamenten uterus:
• Ligamentum latum uteri (peritoneumlaag): ligament over uterus en ruimte ernaast
– Mesometrium: over uterus en zijkant onder ovarium
– Mesoapinx: over tubae en ruimte eronder
– Mesovarium: over ovarium
– Parametrium: ruimte tussen peritoneum (mesometrium). Hier gaan vaten doorheen
,• Ligamentum ovarii proprium: verbindt uterus met ovarium
• Ligamentum suspensorium ovarii: verbindt ovarium met de bekkenwand
– Plooi van bekkenwand naar ovarium
– Bevat a. en v. ovarica, ontspringt uit aorta
• Ligamentum cardinale: om cervix richting zijwand (lateraal)
– Uterina, kruist de ureter → ureter moet heel blijven OPLETTEN bij VUE
– Met a. en v. ovarica
• Ligamentum pubocervicale: cervix naar os pubis (ventral)
• Ligamentum, sacro-uterina: sacrum naar uterus (dorsaal)
• Ligamentum teres uteri (rotundum): uterus naar anulus inguinalis profundus naar labia majora
