Coronaire insufficiëntie + β1 agonist
De β1 agonist zorgt voor een + chronotroop, dromotroop, bathmotroop en inotroop effect. + Chronotroop
en inotroop effect (verhoging van hartritme en verhoging van contractiekracht) leiden tot een toename
van het slagvolume. Daardoor neemt de arbeid en het zuurstofverbruik van het hart toe. Bij coronaire
insuff. kan dus angor ontstaan door een relatief zuurstoftekort.
Diabetici + niet-selectieve β-blokkers
Een β blokker zorgt ervoor dat de glycogenolyse thv de lever en de skeletspier trager verloopt. Daardoor
is er een vertraagd herstel van bloedglucose bij hypoglycemie en een verhoogde incidentie van
hypoglycemie door fysisch inspanning bij diabetici. Ook de orthosympathische alarmtekens van
hypoglycemie (tachycardie en beven) zijn deels geblokkeerd.
Contra-indicaties voor β blokkers:
- Bradycardie
- Sick Sinus syndroom
- Geleidingsstoornissen
- Bestaande hartdecompensatie
- Longoedeem
- Diabetes (je kan wel selectie β1-blokkers geven)
- Astma
- Ziekte van Raynaud
Epilepsie + theofylline (geen absolute contra-indicatie)
Theofylline is een methylxanthine en een competitieve adenosinereceptoragonist. Het wordt omv
bronchodilatatie bij astma gebruikt maar heeft belangrijke neveneffecten, bv. verlaging van de
convulsiedrempel.
Coronaire insufficiëntie + theofylline (geen absolute contra-indicatie)
Theofylline is een methylxanthine en een competitieve adenosinereceptoragonist. Het wordt omv
bronchodilatatie bij astma gebruikt maar heeft belangrijke neveneffecten, bv. tachycardie en verhoging
van de zuurstofnood.
Contra-indicaties voor tricyclische antidepressiva:
- Nauwe-kamerhoekglaucoom
- Urineretentie → owv anticholinerge effecten
- Prostaathypertrofie
- Pylorusstenose
- Hartafwijkingen / recent myocardinfarct → owv cardiotoxiciteit
- Epilepsie → vooral maprotiline: convulsiedrempel↓

, Lithium + verminderde nierfunctie
Lithium wordt renaal geëxcreteerd. Bij verminderde nierfunctie is er verminderde excretie van Lithium
en is er een te hoge bloedspiegel waardoor toxische neveneffecten kunnen optreden. (Hetzelfde treedt
op bij een negatieve natriumbalans)
Zwangerschap + antipsychotica
Afgeraden tijdens hele zwangerschap. Tijdens laatste semester is er risico voor extrapiramidaal
syndroom bij de neonatus
! Angst en slapeloosheid zijn GEEN indicatie voor antipsychotica
Antipsychotica + open hoek glaucoom
Antipsychotica hebben anticholinerge neveneffecten. Daarom zijn de contra-indicaties voor
anticholinergica ook contra-indicaties voor antipsychotica (die sterke anticholinerge neveneffecten
veroorzaken). Anticholinerg effect = mydriase, dit verhindert de afvoer van oogvocht, waardoor de
oogdruk ↑. Dit is al een probleem bij glaucoom, dus is glaucoom een contra-indicatie.
Antipsychotica + prostaathypertrofie
Antipsychotica hebben anticholinerge neveneffecten. Daarom zijn de contra-indicaties voor
anticholinergica ook contra-indicaties voor antipsychotica (die sterke anticholinerge neveneffecten
veroorzaken). Anticholinerg effect= tonus fundus ↓; sfinctertonus ↑, dus gevaar voor urineretentie,
daarorm is prostaathypertrofie een contra-indicatie.
Geneesmiddelen met epileptogene werking:
- Antidepressiva - Cocaïne
- Antipsychotica - Methylfenidaat
- Lithium - Flumazenil
- Theofylline - Bepaalde AB
- Alcohol - Lokale antestheitca
- Barbituraten - Cyclosporine
- Benzodiazepines - Tacrolimu
- Amfetamine
Leverlijden + valproaat
Valproaat heeft hepatotoxische neveneffecten (gevallen van fatale hepatitis beschreven).
Parkinsonisme veroorzaakt door neuroleptica + L-DOPA/D2 agonisten/ amantadine / selegiline
De werking van al die middelen wordt teniet gedaan door de neuroleptica (D2 antagonisten). In geval
van parkinsonisme door neuroleptica moet met antimuscarinica geven.
Astma + morfine
Morfine veroorzaakt bronchoconstrictie als gevolg van histamine vrijzetting, dus mag niet gegeven
worden bij patiënten die lijden aan astma.
Sedativa/hypnotica/antipsychotica + morfine
Beide samen geven is gecontraïndiceerd omdat sedativa/hypnotica/antipsychotica centrale depressie
veroorzaken en morfine dit effect versterkt.
