- Síndromes causados por la alteración de la estructura y función de las arterias que irrigan al cerebro, los
órganos viscerales y las extremidades.
- Aumenta con la edad, más frecuente en pacientes mayores de 70 años. Es una condición casi que del adulto
mayor.
Tratamiento objetivos
- Aliviar los síntomas y mejorar la capacidad para caminar
- Mejorar la calidad de vida
- Reducir la mortalidad total, así como la morbi-mortalidad cardiaca y cerebrovascular
Reducción de factores de riesgo
- Abandono de tabaco
- Presión arterial controlada <140/90
- Lípidos: LDL <100 mg/dl
- Diabetes: HBA1C 6.5 -%
Intervención
- Antiagregación
- Modificación vascular eritrocitaria
- Cirugía (revascularización hasta amputación)
Terapia antiplaquetaria
- Indicado para reducir el riesgo de IAM, ictus o muerte vascular
- ASA: dosis diarias de 75-325 mg (farmacología de la hemostasia y coagulación)
- Clopidogrel: dosis diaria 75 mg (farmacología de la hemostasia y coagulación)
Tratamiento de la claudicación
- Ejercicio físico y rehabilitación
- Cilostazol: 100 mg/12 h
- Pentoxifilina: 400 mg/8 h
- Revascularización: si posterior a 3 meses de manejo consecutivo la discapacidad es persistente o creciente
CILOSTAZOL (anticoagulante)
- Inhiben la FDIII IC
- Aumenta AMPc en plaquetas y ms liso, dando lugar a un efecto antiagregante plaquetario y
vasodilatador.
- Tabletas de 50 mg
- Periodo de latencia entre 2-4 semanas
- Incrementa la tolerancia al ejercicio
RAM
- Proarritmógeno (TV), hipotensión. Diarrea 15%
- Hemorragias
- OJO: inhibidores CYP3A4
PENTOXIFILINA (anticoagulante)
- Inhibe la fosfodiesterasa eritrocitaria: aumenta la actividad del AMPc
- Modifica la membrana eritrocítica
- Reducen la concentración de fibrinógeno
- Aumenta la actividad fibrinolítica
- Disminuye la viscosidad sanguínea
