Inleiding farmacologie:
Farmacologie (onderzoekt de verdeling, effectiviteit en toxiciteit van GM)
→ Farmacokinetiek: studie van het tijdsverloop
van de concentratie van het geneesmiddel in
het organisme.

absorptie, distributie, metabolisatie en excretie
(ADME)

→ Farmacodynamiek: studie van de werking
van het geneesmiddel op het organisme:
concentratie/effect van het GM.

● farmacokinetiek
○ Absorptie: Opname van
farmacon uit
toedieningsplaats in plasma
★ Belang van motiliteit en pH maag-darmkanaal
★ Belang van dikte huid, doorbloeding en lichaamsoppervlak
★ Lichaamsoppervlak in relatie tot lichaamsgewicht: kind > volwassene
→ GM met mogelijke toxiciteit (absorptie ↑): corticosteroïden,
salicylzuur, adrenaline,….
○ Distributie: Farmacon verplaatst zich vanuit plasma naar interstitieel vocht
en/of intracellulair vocht.
★ Groter totale lichaamsvocht en extracellulair compartiment bij
pasgeborene en jonge kind → hogere dosis hydrofiele GM nodig (vb
aminoglycosiden)
★ De mate van eiwitbinding (albumine): let op bij hypoalbuminemie
(wanneer GM vnl eiwitgebonden is) → meer vrije fractie
○ Eliminatie = metabolisatie + excretie: Enzymatische veranderingen (door
groei en ontwikkeling) → COMPLEX metabolisatieproces

★ Metabolisatie: Een GM wordt afgebroken door metabole
leverenzymen (CYP P450) van farmacon tot zijn (inactieve)
metabolieten.
★ Excretie: Het farmacon en zijn metabolieten verlaten het lichaam via
urine, gal of feces.
★ Metabole route kan verschillend zijn in vergelijking met volwassene
★ Metabolisatie: laag bij neonaat, hoog bij 1-5 jarigen, volwassen
capaciteit in puberteit

★ Renale klaring: volwassen waarden tussen 6-12 maand
DUS: leeftijdscategorie steeds nagaan → andere dosering!

○ Hoe beïnvloeden genetische verschillen van metabolisatie-enzymes de
reactie op GM?
