Beste lezer,
Mijn naam is Michael Melisse, 28 jaar oud en samen met mijn vriendin woonachtig in Zevenbergen. Per 1
september 2022 ben ik als leerling HBO-verpleegkunde werkzaam bij Surplus, De Zeven Schakels te
Zevenbergen. Ik ben werkzaam op schakel 6.1, te weten op de ELV-woning. De afdeling bestaat uit negen
kleinschalige woongroepen met variërende doelgroepen: twee woningen somatiek, een woning tweezorg, vijf
woningen PG en een woning ELV (kortdurend verblijf). Op de afdeling vindt samenwerking tussen verschillende
zorgniveaus plaats, namelijk met helpenden (plus), verzorgenden, mbo-verpleegkundigen en een hbo-
verpleegkundige (kwaliteitsverpleegkundige).
Als onderdeel van de module ‘Implementatie’ van de opleiding hbo-verpleegkunde deeltijd is dit document het
eindproduct voor één van de toetsmatrijzen in deze module. U staat op het punt dit eindproduct, welke ook
wel bekend staat als de samenvatting van het implementatietraject, te gaan lezen.
