De schildklier produceert onder andere levothyroxine. Dit is noodzakelijk voor de
ontwikkeling van de hersenenen en het centrale zenuwstelsel, groei en een normaal niveau
van de stofwisseling. Wanneer de hypofyse een signaal geeft, produceert de schildklier
levothyroxine tot er genoeg is. 1% van de vrouwen heeft een te langzame schildklier en 3%
een te snelle. Bij mannen komt dit minder vaak voor.
Thyreomimetica zijn stoffen die de werking van de schildklier nabootsen. Ze worden
toegepast als substitutietherapie bij een hypofunctie van de schildklier. Wanneer de
schildklier niet voldoende levothyroxine produceert, gaat de schildkllier groeien. Dit heet een
struma
Oorzaken hypothyreoïdie schildklier
- Ziekte van Hashimoto
- stille thyreoïditis, duurt 1 tot 4 maanden. Spontaan herstel
- gevolg radiotherapie/chirurgie
- geneesmiddelen als lithium en amiodaron.
Schildklierhormonen moeten 1x per dag een halfuur voor het ontbijt ingenomen worden.
Levothyroxine wordt in de lever omgzet tot liothyronine.
Thyreostatica
Bij een te sterke functie van de schildklier, hyperfunctie. Magere patiënt en uitpuilende
oogbol. Oorzaak kan een te grote schildklier of kwaadaardige celgroei zijn.
Oorzaak hyperthyreoïdie van de schildklier
- Ziekte van Graves
- vergroting van de schildklier
- subacute thyreoïditis, virale infectie
- toxisch adenoom
- geneesmiddelen als lithium, amiodaron en jodiumhoudende contrastmiddelen.
Een hyperfunctie kan behandeld worden door de childklier operatief te verwijderen of te
bestralen met radioactief jodium. Thyreostatica remmen de vorming van het
schildklierhormoon.
22.2 Medicatiebewakingssignalen bij de recept invoer
Effect van hyperthyreoïdie/hypothyreoïdie op de werking van de geneesmiddelen in het
lichaam. Bijvoorbeeld cumarine, digoxine, insuline en bètasympathicolytica.
Thyreomimetica zijn een contra-indicatie bij angina pectoris, hartfalen en een
hartritmestoornis.
Thyreomimetica en thyreostatica geven een interactie met cumarinen waardoor de
bloedstolling beïnvloed wordt.
