In het kader van mijn opleiding tot hbo-v verpleegkundige, toon ik u middels dit document
mijn kennis met betrekking tot participatie omtrent de stroke patiënt. Ik zal aantonen dat ik
over voldoende kennis beschik over dit onderwerp, en deze kan toepassen in de praktijk.
Dit onderwerp heb ik gekoppeld met mijn verslag ‘zelfmanagementondersteuning bij de
acute stroke patiënt’, luk 4.1. In dit verslag ga ik door op de casus van patiënt H. Een 63-
jarige man met een intraventriculaire hersenbloeding. Ik beschrijf wat er allemaal komt kijken
bij het participeren met een chronische ziekte.
Ik bedank patiënt H. en zijn naasten voor hun vertrouwen in mij gedurende de opname.
Daarnaast bedank ik mijn netwerk voor hun hulp, mijn medestudenten voor hun tips, mijn
