- Dwarslaesie = beschadiging van het RM (gewoonlijk transversale) en/of van de in het wervelkanaal
verlopende zenuwwortels (o.a. cauda equina)  continuïtetisonderbreking van opstijgende en
afdalende neurale structuren.

, SYMPTOMATOLOGIE

1. Motorische uitval
 Bij onderbreking afdalende systemen  verlamming distaal van de laesiehoogte
 Spastische of stijve verlamming (UMNL):
 Reflexboog nog intact (maar supraspinaal niet beïnvloedt)
 Hyperreflexie, pathologische reflexen, verhoogde spierspanning
 EERST: flexiepatronen benen dominant (withdrawal-reflex)  fasisch
(‘spasmen’)
 LATER (na 3 à 6 maand): extensiereflexen dominant  tonisch
 Slappe verlamming (LMNL):
 Reflexboog beschadigd
 Areflexie, atonie
 -plegie (para of tetra)
 - parese (para of tetra)  bij verminderde F in het lidmaat
2. Sensibele uitval
 Bij onderbreking  sensibele prikkels thv. of onder letsel worden niet bewust waargenomen
 Afhankelijk van lokalisatie  ≠ gevoelsbanen selectief getroffen
 Vitale sensibiliteit:
 Pijn-
 Tast-
 Temperatuurzin
 Gnostische sensibiliteit:
 Fijne tastzin
 Vibratiezin
 Houdings- en bewegingsgevoel
 Gevolgen? Decubitis (druknecrose, doorligwonde)
Verhoogde tonus
Seksuele disfuncties
Incontinentie
3. Vegetatieve stoornissen
 Stoornissen in bloeddrukregeling
Blaas- en darmfunctie
Seksuele functies
Ademhaling
Thermoregulatie
 VB. bij een cervicale of hoog thoracale laesie  PS werkt nog via N. vagus, maar OS werking
kan verminderd zijn onder niveau letsel
 OS (T1-L2) / PS (hersenstam + S2-S4)
4. Spinale shockfase
 Tijdelijke deactivatie van exciterende impulsen vanuit het getroffen weefsel en de
omliggende delen van het CZS
GEVOLG? Volledig functieverlies thv. beschadigde en niet beschadigde segmenten
onder het niveau van de laesie (totale uitval onder het letsel)
 Terugkeer reflexen is einde shockfase
 Duur shockfase belangrijk!

 Binnen 1 – 6 u: Delayed plantar response (DPR)
