Pre-Assessment Instructions - ANS-if client has elected to have full or partial fitness
assessment
clothes appropriate for exercise
do not eat, drink caffeinated liquids or alcohol, or smoke within 2 hours
do not engage in strenuous exercise within 6 hours
Informed Consent - ANS-Required
, describes assessment and outlines client responsibilities in the safe administration of
procedures
read, understood, & signed prior to any CSEP-PATH process
not a waiver, EP is professionally responsible at all times
Under 18 - must have parent or guardian sign youth consent form
signing must be witnessed by adult third party

client must read and answer questions without interpretation from CPT
Signed in ink & witnessed by third party
answers determine if client is within scope, requires clearance
valid for 12 months unless health condition changes
apparently healthy = all no answers on page 1
stable health condition (low risk) = one + yes answers on page 1, all no answers to
questions on pages 2-3, no responses to questions 2, 6, and 7 - yes to any of these
suggests potential unstable condition and clearance is warranted
Unstable health condition = one or more yes answers on page 1; one or more yes
answers on pages 2-3 = CSEP-CEP
person who experiences chest pain during PA - refer to physician regardless of answers
on pages 2-3
Resting Heart Rate Protocol - ANS-sit in comfortable char with arm supported and feet
flat on floor ~5 mins
Stethoscope, radial or carotid artery palpation
15 second count with first beat as 0
read RHR to nearest beat in 15 s - multiply by 4 = BPM
RHR = 99 BPM or less - proceed
RHR > 99 BPM - ask client to sit quietly for additional 5 minutes and repeat
RHR still > 99 - do not proceed with active assessments & refer to physician
Resting Blood Pressure - ANS-the force of blood against the walls of the arteries
created by the heart as it pumps blood to the body
Systolic BP - first perception of sound
Diastolic BP - sounds cease
same body position as resting HR
SBP 144 mmHg or less & DBP 94 mmHg or less - proceed
SBP > 144 mmHg and/or DBP > 94 mmHg ask client to sit quietly for additional 5 mins
and repeat, if still above limit do not proceed and refer to physician
