De indeling en vormen van pijn – definities

o Er bestaan verschillende indelingen en vormen van pijn:
- Nociceptieve versus neuropathische versus nociplastische pijn
- Somatische versus viscerale pijn
- Doorbraakpijn
- Opioïd geïnduceerde hyperalgesie
- Totale pijn

Nociceptieve versus neuropathische versus nociplastische pijn

o Nociceptieve pijn
- Veroorzaakt door weefselbeschadiging  deze pijn is meestal goed behandelbaar
met paracetamol, NSAID's (Non-Steroidal Anti-Inflammatory Drugs) en/of
opioïden
- Bv. je snijdt in je vinger, …

o Neuropathische pijn
- Tgv beschadiging vh perifere of centrale zenuwstelsel door compressie of ingroei
door tumor, door bestraling of na operatieve ingrepen
- Zenuwpijn  bv mensen met cva die bloeding hebben thv hersenstam, die
bloeding kan drukken op de zenuw vd hersenstam
- Anti-epilectica en antidepressiva gaan geven tegen deze pijn

o Centrale pijn
- Is neuropathische pijn die veroorzaakt w/ door beschadiging vh centrale
zenuwstelsel
- Bv. na een CVA of bij multipele sclerose.

o Nociplastische pijn
- Ontstaat door veranderingen in de nociceptieve processen zonder dat er nog
bewijs is van actuele schade van weefsel of zonder bewijs van schade of ziekte vh
zenuwstelsel
- Letsel heeft zich ingenesteld  pijn is er maar niet altijd constant

Eigenschappen van neuropathische pijn:

o De pijn w/ vaak als brandend, schietend en/of stekend ervaren.

o Vaak gepaard met sensibiliteitsstoornissen:
- Pijngewaarwording kan ook versterkt zijn (hyperalgesie)
- Soms treden pijnklachten op als gevolg v/e prikkel die normaliter niet tot een
pijnsensatie leidt (allodynie)
- Huidaanraking is meestal pijnlijk (hyperesthesie)

o De plaats waar de pijn w/ gevoeld hangt af vd plaats waar de zenuwbeschadiging is
opgetreden
o Minder gevoelig voor paracetamol, NSAID's en opioïden dan nociceptieve pijn
- Vaak antidepressiva en anti-epileptica nodig

 Ca. 65% vd pijn bij patiënten met kanker is nociceptief en 10% is neuropathisch. In 25% vd
gevallen is er sprake van mengvormen.

 Zenuw gebonden pijn = hypo of hyperesthesie of disesthesie
 Antidepressiva: Redomex – Lyrica – Neurontin
 Allodynie = zaken die normaal geen pijn doen gaan nu wel hard pijn doen door wijziging in
sensibiliteit tgv beschadiging zenuwbaan

Somatische pijn versus viscerale pijn

o Somatische pijn
- = nociceptieve pijn die uitgaat van huid, bindweefsel, spierweefsel of bot 
meestal duidelijk gelokaliseerd en scherp, stekend of kloppend van karakter.

o Viscerale pijn
- = nociceptieve pijn die uitgaat vd ingewanden vd borst of de buik  meestal niet
duidelijk gelokaliseerd en vaak drukkend of krampend van karakter.
- Viscerale pijn w/ soms gevoeld in het dermatoom  op hetzelfde
ruggenmergniveau projecteert

o 'referred pain'
- Bv. pijn in de schouder tgv levermetastasen met ingroei in het leverkapsel en het
diafragma
- Bv. pt met myocardinfarct  pijn situeert zich niet thv het hart maar straalt uit
naar arm

Doorbraakpijn

o  plotseling optredende, tijdelijke, vaak hevige toename van pijn die optreedt bij
bestaande chronische pijnklachten

o Komt voor bij 50- 65% vd pt met kanker, duurt gem. 30 min. en gem. 4 X/dag

o Kan invaliderend zijn en leiden tot stemmingsstoornissen en angst.
- De medicatie die w/ gebruikt voor de behandeling van doorbraakpijn w/ ook wel
doorbraakmedicatie genoemd.

o Onderscheid tussen 3 types doorbraakpijn:
1. Incidentele pijn:
 Doorbraakpijn die uitsluitend of hoofdzakelijk optreedt bij bepaalde
bewegingen of houdingen (voorspelbaar)

2. 'End of dose pain':
 Doorbraakpijn die optreedt kort voor de volgende gift vh analgeticum

3. Spontane doorbraakpijn:
 Doorbraakpijn die optreedt zonder aanwijsbare aanleiding (niet-voorspelbaar)

 Plan B bij doorbraakpijn = Paracetamol 3g/dag om de 8 uur + zo nodig extra Tramadol
50mg/dag

 Dafalgan mag max. 4g/24uur => levertoxisch

 Als je in vpk dossier ziet: dafalgan 3X/dag en zo nodig… = meestal doorbraakpijn

Opioïd-geïnduceerde hyperalgesie

o Indien toediening van opioïden leidt tot hyperalgesie en toename van pijn. Dit kan
optreden zowel bij zeer hoge doseringen of snelle dosisverhoging.
- Pijn tgv van opiaten  sommige mensen die onder opioïten staan reageren er
heel slecht op  krijgen net meer pijn  opioietrotatie

Totale pijn

o Ook primair somatisch bepaalde pijn w/ in hoge mate beïnvloed door psychosociale
en/of levensbeschouwelijke factoren.
o Voor dit allesomvattende concept van pijn w/ ook de term 'totale pijn' gebruikt.
- Total body pain = overal pijn zonder duidelijk aanwijsbaar oorzaak
 naar pijnkliniek
