Lokale behandeling:

2 Diepe veneuze trombose (DVT)
2.1 Pathofysiologie
= aandoening waarbij diep gelegen venen een trombus (stolsel)

2.2 Risicofactoren
Hogere leeftijd (60+)
Voorgeschiedenis veneuze trombo-embolie
Erfelijkheid
Gebruik hormonale contraceptie
Zwangerschap
Dehydratatie (hemoconcentratie)
Weefselnecrose (afstervend) en maligniteit (kwaadaardig proces)
Immobilisatie (ziekte, coma, lange vlieg- bus en autoreizen)

2.3 Symptomen
Aspecifiek:
Zwelling
Zwaartegevoel
Doffe pijn (kloppend)
Klassieke klinische tekenen:
Oedeem (vochtophoping)

2.4 Diagnose (grondig lezen)
Anamnese
Lichamelijk onderzoek
Echo-duplexonderzoek (van bloedvaten + meten snelheid in aders)
Bloedonderzoek:
o D-dimeren opsporen : verhoogde waarde activatie v
stollingssysteem , niet bewijzend voor DVT infectie kan ze ook
doen stijgen
o Lage waarde uitsluiting
2.5 Behandeling
Preventief werken !!
Na ingreep:
o Snelle mobilisatie
o LMGH
o Eventueel TEK (therapeutische elastische kousen) klasse 1
Niet medicamenteuze behandeling:
o Voorlichting : oorzaak en aanwezige risicofactoren bespreken,
bedrust niet nodig
o Compressietherapie
o TEK klasse III gedurende 2 jaar
o Positionering : hoog leggen v been zal oedeem doen afnemen
Medicamenteuze behandeling:
o Laagmoleculair-gewichtsheparine (LMGW) in therapeutische dosis
hogere dosis dan preventieve dosis
o DOAC: directe orale anticoagulantia
o Bij uitgebreide DVT : trombolyse bloedklonter opgelost door
medicatie via katheter
2.6 Complicaties
Longembolie : ong helft v ZV met DVT , 70% van ZV met longembolie
wordt DVT vastgesteld gaan vaak samen veneuze trombo-embolie
(VTE)
