HOOFDSTUK 1: DEPRESSIES
INLEIDING
Een depressieve episode -> gedurende min 2 weken bijna dagelijks ten minste 5 symptomen
- Sombere stemming
- Verlies van interesse of plezier
- Gevoelens van waardeloosheid of schuld
- Suïcide gedachten of pogingen
Behandeling:
- Psychotherapie
o Aandacht voor relationele context -> wat is er met die persoon gebeurd
o Luisteren
- Farmacotherapie
o Normaliseren van stemming, intresseverlies, schuldgevoelens
o Werking na 1-2 weken, op volle kracht na 4 weken, nevenwerkingen na enkele uren!!!
§ Niet zo communiceren -> langzamerhand gaat u beterschap voelen
o Placebo-effect is groot
o Patiënten kunnen al beterschap voelen na enkele dagen
o Ernst van de depressie bij de aanvang heeft invloed op resultaat -> depressie erger in het begin -> beter
resultaat bekomen
TRICYCLISCHE ANTI-DEPRESSIVA (TCA)

Amitriptyline ** (Redomex, Tryptizol) Nortiptyline (Nortrilen)
- Zeer sedatief! Dit kan wel nuttig zijn in de
context van depressie
- Rijden met wagen kan dan bijvoorbeeld niet
Clomipramine (Anafranil)
- Minder sedatief, eerder als neutraal

behandelen)
Obsessieve-compulsieve stoornis
Indicaties - Clomipramine
Enuresis nocturna -> bedwateren (hebben een anti-cholinerg effect)
Ideale moment om te - Amitriptyline, Imipramine
geven: ’s avonds omdat Chronische pijn
sedatie veroorzaakt! - Neuropatische pijn: aminotryptiline -> lage dosis!
Profylactische behandeling van migraine
- Aminitryptilline
Parasympathicolytische effecten
- Niet wensen bij ouderen -> centrale nevenwerkingen (dementie symptomen)
- Verminderde therapietrouw
Anti-noradrenerge werking
- Ouderen -> leidt tot vallen
Nevenwerkingen - Minder kans bij nortryptilline
Hartgeleidingsstoornissen
Anti-histaminerg -> sedatie
Verlaging convulsiedrempel -> patiënten met epilepsie makkelijk een epilepsie aanval
Bij plots staken -> ontrekkingsverschijnselen
Hartpatiënten
- Recent myocardinfarct
- Hartritme en geleidingsstoornissen
- Angina pectoris
Contra-indicaties - Hartfalen
Nauw kamerhoekglaucoom (anticholinerg effect)
Prostaathyperplasie (anticholinerg effect)
Epilepsie
Bipolaire stoornis zonder stemmingsstabilisator
- Accidentieel of intentioneel
Intoxicatie - 2 g kan fataal zijn!
Als patiënt suicide risico heeft -> niet de volledig doos meegeven enkel een stripje

Sympaticomimetica:
- Fenylefrine oraal -> stoppen
à Effect zal stijgen
- Indirect werkende -> respons dalen (vb: efedrine , amfetamine)
Interacties Indirect werkend: werkt indirect op de R -> presynaptisch opgenomen en gaan vanuit vesikels NA
vrijstellen -> opname van efedrine gaat verminderen door gebruik TCA
Clonidine:
- TCA blokkeren opname clonidine
Desensitisatie van centrale presynaptische alfa 2 receptoren (- FB -> minder NA vrijgave)
Cholinesteraseremmers met centrale werking
TCA = een groep die centraal anti-cholinerg werkt
TCA bij ouderen - Veranderde kinetiek
- Bij elke oudere patient verminderde nierfunctie
- Gevoelig voor anticholinerg nevenwerkingen
- Risico op vallen wegens alfa blokkerende werkingen
- Voorkeur= nortriptyline
- Helft van normale aanvangsdosis door veranderde kinetiek
- Max effect na 6 weken (op volle kracht)
