1.1 Chirurgisch gesloten wonde (primair gesloten):
- Chirurgisch gesloten, niet lekkende (droge) wonde = steriele wonde, waarbij door
chirurgische handeling gladde wondranden steriel gesloten zijn. Genezing is
ongestoord: enkel smalle huidsnede zichtbaar, huidranden liggen volledig tegen elkaar
1.2 Chirurgisch gesloten wonde (primair gesloten) met lekkage:
- Chirurgisch gesloten, lekkende wonde: chirurgisch wondvlakken gesloten, maar waarbij
verlies van wondvocht optreedt
1.3 Chirurgisch gesloten wonde met drainage:
- Gesloten chirurgische wonde waarbij door chirurgische handeling wondvlakken
gesloten zijn, met aanwezig
o Vb: Drain/wiek om overtollig wondvocht te laten afvloeien
2. CHIRURGISCH OPEN GELATEN WONDEN
- Wonden waarbij door chirurgische handeling weefsel en huid openblijven
3. OPEN GEKOMEN WONDEN
- Wanneer huidincisie niet door hechtingen gesloten blijft, maar openscheurt, spreekt
men van wonddisruptie/evisceratie. Er ontstaat een open wonde
4. VERZORGING VAN DE CHIRURGISCHE GESLOTEN WONDE / B1-HANDELING
- Wonde wordt in operatiezaal ontsmet & afgedekt met steriel verband, dit verband blijft
ter plaatse tot 1ste controle na ingreep: ten vroegste 48u na ingreep plaats, soms zelfs
pas na 7-10 dagen wanneer hechtingen verwijderd worden
- Nieuw verband wordt nog aangebracht op wonde, om: patiënt gerust te stellen en
kwetsbare wonde nog te beschermen tegen stoten, ... > zeker verdere wondheling niet
te verstoren
- Frequentie van wondverzorging wordt bepaald op basis van afgesproken criteria.
- Verband van gesloten wonde wordt dagelijks geïnspecteerd. Deze wonde hoeft in
principe geen verzorging tenzij verband verzadigd is, tekens bloed vertoont, buiten uit
bevuild is, loskomt of indien patiënt koorts heeft of lokale klachten.
5. VERZORGING VAN CHIRURGISCHE GESLOTEN WONDE MET LEKKAGE OF
DRAINAGE/B1- HANDELING
- Doel: wondheling bevorderen, wondomgeving beschermen voor irritatie, wondvocht
absorberen
- VP technische handeling: Zie studentenportaal: procedure: chirurgisch gesloten wonde
met lekkage
- Rapportage: Volgens TIME en wound care cirkel
