1.1. ADME

1.2. Calcium distributie
99% van calcium in het skelet onder de vorm van hydroxyapatiet , de rest in weke weefsels,
extracellulair vocht en het plasma
Plasma calcium komt voor onder 3 vormen :
- Geïoniseerd calcium : 50% van totaal biologisch actieve fractie
- Eiwitgebonden calcium : vooral albumine : 45%
- Calcium gecomplexeerd met anionen zoals citraat, fosfaat, lactaat en bicarbonaat : 5%
diffundeerbaar maar biologisch niet actief
Calcium gemeten in routine laboratoriumbepalingen wordt gemeten in plasma en is het
totale calcium partieel gebonden aan eiwitten
Overige fractie is diffundeerbaar , deel gecomplexeerd met anionen , overige is vrij
beschikbaar enkel deze laatste is biologisch actief en moet in homeostase zijn

1.3. Calcium bepaling
Totaal calcium
- Spectrofotometrisch – routine chemie analyzers
- AAS – referentie methode
- Totaal calcium gecorrigeerd voor albumine concentratie
o Naast pH effecten zullen verandering in albumine concentratie de totale Ca
concentratie beïnvloeden indien totale calcium gemeten wordt , kan dit
aanleiding geven tot foute interpretatie in geval van hypoproteïnemie en
hyperproteïnemie
o k 40g/L wordt gezien als normaal
