, Inleiding
In het verslag gaat u lezen hoe ik samen met een cliënt begeleidingsdoelen formuleer en hoe de
cliënt de begeleidingsdoelen kan behalen. Ik ben werkzaam bij stichting Odion. Odion ondersteunt
1700 kinderen en volwassenen. Dit doen wij op 63 woon-dagbestedingslocaties, leerwerkbedrijven
en via ambulante ondersteuning in de regio Kennemerland, Zaanstreek en Waterland. Ik maak
gebruik van de methodieken die Odion hanteert om een begeleidingsplan om te stellen.

Voor deze casus heb ik cliënt Sara gebruikt. Sara is 30 jaar oud en woont op een woonvorm. Ik ben
haar persoonlijk begeleider. Sara en ik zijn in gesprek gegaan om een begeleidingsplan te maken,
zodat we haar de juiste ondersteuning kunnen bieden en dat het aansluit bij de wensen en behoeftes
van Sara.
