TYPE 1 DIABETES MELLITUS (T1DM)
DEFINITIE
Reductie in het aantal bèta-cellen (>75%) → insulinetekort
- T-cel gemedieerde immuunreactie gericht tegen β-cellen → insulitis
o Met β-cel antilichamen
- Associatie met erfelijk materiaal en veroorzaakt door een combinatie van genetische- en omgevings-
factoren (Viraal? Dieet? Toxica? …) => multifactorieel
TYPEN
- Klassieke type 1 DM
o Onset: meestal voor de leeftijd van 40 jaar (juveniele onset)
▪ 50% voor 16j
o Symptomen: ernstig en snel progressief: polydipsie, polyurie, vermagering
- LADA = late auto-immune diabetes of the adult
o Auto-immune destructie van de B-cellen verloopt langzamer
o Iemand van 45j die bij u komt, kan dus wel gewoon type 1 hebben → leeftijd is geen 100%
