Pharmacological:
Dyslipidemia:
Lipitor 10mg, PO Daily Increase to Lipitor 40 mg, PO, Daily, QHS
-Recommendation to keep LDL <100; we don’t know the LDL level but intensive lipid lowering affects is
recommended; I chose 40 since we don’t know the LDL
Blood Pressure: 158/90
-Losartan 100 mg, PO, Daily Losartan 100 (0r 150mg), PO, daily + Hydrochlorothiazide 25mg, PO, daily
-Recommendations to keep BP <140/90 post CVA; already on a BP medication; should increase dose, add
another or both
Anticoagulation/Anti-Platelet:
-I think she needs something, but the guidelines that I’ve found and posted seem to be very specific
regarding the cause of the CVA, which we don’t know… unless we discuss different treatment depending
on different causes; ex… a-fib, carotid stenosis, etc…
Glucose: Level looks ok; screening is recommended
Obesity: - BMI is 33.7 which is considered obese
All patients with CVA should have a BMI screening
-Recommendation: Nutrition therapy
----3-4 days per week of moderate to vigorous exercise; 40 min each
Physical Tolerance: says no MS weakness, so she should be able to do activity
Nutrition: Albumin- 3.9 – WNL
-Protein- 6.8 – WNL
-Constipation may be an intolerance level or types of foods each since nutrition seems ok
-Nothing mentions a swallow eval- but she must be eating something if she’s gaining weight

Glucose – 94- WNL
Bun 12- WNL
Creatinine- 1.1- WNL
Sodium- 138- WNL
Potassium- 4.2- WNL
