Gedilateerde CMP
Er ontstaat een belangrijke dilatatie (volumetoenamen) van
de 4 kamers zonder dat er een bekende oorzaak voor is
Dikte van de spierwand blijft normaal, volume is
enkel toegenomen
Licht microscopisch:
- Myocyten degeneratie
- Hypertrofie en atrofie van de myocardvezels
- Interstitiële en perivasculaire fibrose
OORZAKEN:
- Idiopathisch (geen duidelijke oorzaak/ onbekend) → meest frequent
- Inflammatoir
o Infectieus (viraal) → vb covid
o Niet-infectieus
▪ Peripartum CMP → hartspierziekte aan het einde van de zwangerschap
▪ Bindweefselziekte
▪ Sarcoidose
- Toxisch
o Ethanol (alcohol) → kan terminale CMP veroorzaken, typisch kenmerk is dat ze omkeerbaar is →
door alcohol stop kan het hersteld worden
o Adriamycine = chemotherapeticum → kunnen cardiale toxiciteit veroorzaken op enkele maanden tijd
- Metabool
o Hypothyroidie → te traag werkende schildklier
zwangerschap myopathie: systemische overbelasting van volume voor de vrouw -> zwangerschap & hart is een
complex gegeven -> na zwangerschap zal deze volumetoename terug verkleinen en normaliseren -> bij sommige
vrouwen blijft dit behouden en zelfs verergeren -> kan zeer uitgesproken worden -> peripartum CMP (jonge moeders
op UZA)
Gedilateerde CMP → te groot hart
- Meet de diameter van het hart op zen breedste
- Meet de diameter van de thorax
Wanneer het hart groter is dan de helft van de thorax is het abnormaal
, Hypertrofische CMP (HOCM)
HOCM = hypertrofe obstructieve cardio myopathie
Specifiek symptoom:
Normaal myocard Hypertrofe cardiomyotrofie
Indien je een chaotisch patroon hebt, kan je cirkel
stromen creëren → ronddraaiend front → plotse dood
OORZAAK: soms is de oorzaak een mutatie
Van sommige weten we de mutatie al maar er komen elke dag nog steeds bij
patiënt in ziekenhuis met dit fenomeen?
