CHAPTER 16.

A drug-induced loss of consciousness - Ans✔✔-general anesthesia

A loss of sensation in a region of the body - Ans✔✔-regional anesthesia

An anesthetic agent is injected in the subarachnoid space, into the cerebral spinal fluid
(CSF), in the patient's spinal canal for surgeries performed below the upper abdomen -
Ans✔✔-spinal anesthesia

An anesthetic agent is injected in the epidural space. A small catheter may be placed for a
continuous epidural. An epidural can also remain in place after surgery to assist with
postoperative pain - Ans✔✔-epidural anesthesia

An anesthetic agent is injected directly into the area around a nerve to block sensation for
the region the surgery is being performed. Commonly used for procedures on the arms or
legs - Ans✔✔-nerve block

Anesthesia service where the patient is under light sedation or no sedation while undergoing
surgery with local anesthesia provided by the surgeon. The patient can respond to
purposeful stimulation and can maintain his own airway - Ans✔✔-monitored anesthesia care
(MAC)

one lung is ventilated, and the other lung is collapsed temporarily to improve surgical access
to the lung or thoracic cavity - Ans✔✔-one-lung ventilation

describes when a cardiopulmonary bypass (CPB) machine is used to function as the heart
and lungs during heart or great vessel surgery - Ans✔✔-pump oxygenator

,describes organs within the peritoneal cavity. These procedures may be performed in both
the upper and lower abdomen - Ans✔✔-intraperitoneal

describes the anatomical space in the abdominal cavity behind or outside the peritoneum -
Ans✔✔-extraperitoneal, retroperitoneal

usually extensive, complex, and intended to correct a severe health threat such as a rapidly
growing cancer - Ans✔✔-radical surgery

a drug induced depression of consciousness. The patient can respond to verbal direction or
by light tactile stimulation. Interventions are not required to maintain the patient's airway. A
surgeon may use this sedation for a surgical procedure and code for it as well as the surgery
- Ans✔✔-Moderate (conscious) sedation

occurs when an anesthesiologist is involved in two, three, or four anesthesia procedures at
the same time, or a single anesthesia procedure with a qualified anesthesia resident, CRNA,
or anesthesiologist assistant - Ans✔✔-medical direction

Procedures commonly carried out in addition to the primary procedure performed. Add-on
codes may not be reported alone and are identified with a + sign - Ans✔✔-add-on codes

Time begins when the anesthesiologist (or anesthesia provider) begins to prepare the
patient for the induction of anesthesia and ends when the anesthesiologist (or anesthesia
provider) is no longer in personal attendance - Ans✔✔-anesthesia time

A catheter inserted into an artery. It is used most commonly to measure real-time blood
pressure and to obtain samples for arterial blood gas - Ans✔✔-arterial line

Value assigned to anesthesia codes for anesthetic management of surgery and diagnostic
tests. Base unit values will vary depending on the difficulty of the surgery or diagnostic tests,
and the management of anesthesia - Ans✔✔-base unit value

A technique used to take over temporarily the function of the heart and lungs - Ans✔✔-
cardiopulmonary bypass (CPD)

,A catheter placed in a large vein such as the internal jugular, subclavian, or femoral vein with
the tip of the catheter close to the atrium, or in the right atrium of the heart - Ans✔✔-central
venous catheter

Technique used in general anesthesia to reduce blood pressure to control bleeding during
surgery. Watch anesthesia record for notes regarding - Ans✔✔-controlled hypotension

Direct measurement of the blood pressure in the right atrium and vena cava. CVP reflects
the amount of blood returning to the heart and the ability of the heart to pump the blood from
the right heart into the pulmonary system - Ans✔✔-central venous pressure (CVP)

based on the physician fee schedule. Payments are made under the Relative Value Unit,
rather than by Conversion Factor. Time is not a consideration for payment. Examples are
arterial lines, CV line, emergency intubation, and Swan-Ganz catheter insertion - Ans✔✔-flat
fee

Implies a temperature of 20 degrees centigrade or less - Ans✔✔-hypothermic circulatory
arrest

A flow directed catheter inserted into the pulmonary artery. PACs are used to measure
pressures and flows within the cardiovascular system (for example, Swan-Ganz) - Ans✔✔-
pulmonary artery catheter (PAC)

Circumstances significantly affecting the character of an anesthesia service. These add-on
procedures may be reported only with anesthesia codes. More than one may be reported, if
applicable. May not be reported separately when a code descriptor already indicates the
circumstance. Do not use these modifiers for Medicare Beneficiaries - Ans✔✔-qualifying
circumstances

A unit measure used to assign a value to services. It is determined by assigning weight to
factors such as physician work, practice expense and malpractice expense - Ans✔✔-relative
value unit (RVU)

Anesthesia provider avoids an area where the surgeon is working (usually on procedures
around the head, neck or shoulder girdle). Be sure that there is a minimum of base 5
assigned for anesthesia requiring field avoidance - Ans✔✔-surgical field avoidance

Deliberate reduction of a patient's total body temperature, which reduces the general
metabolism of the tissues. Evaluate the anesthesia record for notes regarding total body

, hypothermia. Generally, temperature is reduced 20 percent to 30 percent below a patient's
normal temperature. This may not be reported separately when code indicates it is included -
Ans✔✔-total body hypothermia

Unusual Anesthesia—This modifier describes a procedure usually not requiring anesthesia
(either none or local), but due to unusual circumstances, is performed under general
anesthesia. For example, a pediatric patient may require general anesthesia for the surgeon
to perform a procedure not requiring anesthesia under usual circumstances - Ans✔✔-
Modifier 23

Anesthesia by Surgeon—This modifier is appended to CPT® surgery codes when the
surgeon provides regional or general anesthesia but does not apply to local anesthesia. This
modifier is not reported with anesthesia CPT® codes and not reported by anesthesia
providers - Ans✔✔-Modifier 47

Discontinued Procedure—This modifier is reported by the provider to indicate that a
procedure was started and, due to extenuating circumstances, was discontinued. Check with
payer. Usually, modifier 53 is used with surgical and diagnostic CPT® codes. Many payers
do not require modifier 53 for anesthesia CPT® codes. Anesthesia reports the anesthesia
code and the time for the discontinued procedure after the start of anesthesia. If the payer
requires modifier 53, it is reported as the last modifier after the anesthesia modifiers -
Ans✔✔-Modifier 53

Distinct Procedural Service—This modifier indicates a procedure or service is distinct or
independent from other non-E/M procedures. Documentation must support a different
session, procedure, surgery, site, organ system, incision/excision, or injury. This modifier is
often appended to postoperative pain management services to disassociate them from the
anesthesia administered during surgery - Ans✔✔-Modifier 59
