Chief complaint “I feel like my whole world is crashing!”
History of present illness:
Isabella is a 19-year-old American Caucasian female who presents with her mother due to
feelings depression and hopelessness. She reports being stressed out about school. She has her
final group presentation coming up next week and she worries about “messing things up.” She
reports restlessness, irritability and difficulty sleeping at night as she is too consumed with
negative thoughts about the presentation. She fears that she will mess everything up and would
be judged by her group as the reason for their low grade. She mentioned that she once had a
terrible humiliating experience in 8th grade when she was presenting her final project to her
class where she felt so anxious in the middle of her presentation that she started sweating and
her voice started shaking. She then blanked out and immediately exit the classroom without
finishing her presentation. She stated that this experience was very embarrassing for her, and
she has not gotten over it yet. She has no interest in social activities at school and would turn
down invitations from classmate. She belief she does not “belong” and would be called “lame
or dumb.” If she dares tries to hang out with her classmate. She claims that the other girls in the
class are just “too cool” for her. She spends most of her time alone in her room playing video
games on her cell phone or with her cat. Isabella’s mother also expresses her concern about
Isabella’s academic achievement as Isabela occasionally misses school and comes up with
excuses why she cannot make it to class. She worries that her daughter might not make it
through this school year if her behavior isn’t changed.
Social History:
- Denies alcohol use.
- Denies tobacco, marijuana, or any illicit drug use.
- Spends majority of her leisure time in her room with electronic device or with her cat
- Currently single and not dating
- Has one friend from third grade whom she no longer keeps in touch with due to
relocation.
Family History:
- Mother: HTN, Type II DM, HLD, Crohn’s disease
- Father: CAD, Bipolar disorder, DVT, HLD
Past Medical History
- 3 years history of migraines
- Anemia
- Gastritis
- Denies other significant past medical history or surgery.

- Iron 325 mg daily
