Normale fysiologie van pijn (4 stappen)
1. transductie: het ontstaan van pijnsignaal door prikkeling van vrije zenuwuiteinden thv perifere
pijnreceptoren (nociceptoren)
● nociceptoren = hoogdrempelige receptoren
○ stimulus moet van hoge intensiteit zijn, voordat nociceptoren geactiveerd worden en
actiepotentialen uitsturen
● nociceptieve stimuli
○ fysische factoren: druk, rek, warmte, koude
○ chemische factoren: ontstekingsmediatoren (bv. prostaglandines, histamine, bradykinine,
serotonine, cytokines) = chemical soup

⇒ proces van pijn is onafhankelijk van het type stimulus dat de pijn veroorzaakt

2. transmissie: de voortgeleiding van de actiepotentiaal via geactiveerde zenuwvezels
● van periferie naar dorsale hoorn
● van dorsale hoorn naar cortex (ascenderende banen)

3. modulatie: veranderingen in de reactie op het pijnsignaal
● snelle aanpassingen van het lichaam om pijnsignaal te onderdrukken/verhogen
○ meestal uitvergroten: verdedigingsmechanisme, waarschuwing
● op elk niveau !

4. perceptie: sensatie van pijn wanneer signaal aankomt ter hoogte van cortex
● integratie en proprioceptie → pijngewaarwording

● nocireceptoren zitten thv verschillende soorten receptoren
○ thermoreceptoren, chemische receptoren, baroreceptoren, elektromagnetische receptoren
● voorwaarde: hoge intensiteit, potentieel schadelijk

nociceptieve stimulus: kan het nociceptieve systeem activeren
algogene stimulus: kan een pijnlijk gevoel veroorzaken


Nociceptoren
von Frey (1896)
● ontdekte dat er bij bepaalde aandoeningen een afwijkende drempel voor het detecteren van druk en
afwijkende drempel waarop druk pijnlijk wordt is wanneer druk wordt uitgeoefend op de huid
● Von Frey haartjes: het detectievermogen testen
○ haartjes van verschillende dikte op de huid duwen, om de G-kracht op de huid te berekenen




Thermoreceptoren
brede range (-10 tot 60°C) van temperaturen die het lichaam kan onderscheiden
● 30°C op huid: geen perceptie, de nulwaarde
● bij stijgende temperatuur: activatie van thermoreceptoren die warmte voelen (Trpv4)
○ 40°C: activatie van nociceptoren, gevoel van warmte valt weg en gevoel van pijn overheerst
(geen onderscheid meer tussen koude of warmte)
● bij dalende temperaturen: activatie van thermoreceptoren die koude voelen
○ -10°C: activatie van nociceptoren, gevoel van pijn
○ grotere range, koude gevoel blijft beter behouden
