1. Potential for Complications of Diagnostic
Tests/Treatments/Procedures: Use precautions to prevent injury

precautions to help reduce the likelihood of injury or complications.
One of the most important precautions for this patient is to prevent
infection. The patient may also need to be on bleeding precautions,
depending on their lab results. Rationale - For a patient who is
undergoing chemotherapy, they are at an increased risk for
developing infections because their immune system is weakened.
Chemotherapy can also increase the risk of thrombocytopenia,
which increases the risk of bleeding, because their body is not
producing platelets to help clot the blood.
2. Laboratory values - Monitor client laboratory values. For a client
who is currently undergoing outpatient therapy, it is important for
the nurse to know what the patient's lab values are, especially
regarding the CBC. Rationale - Patient's who are undergoing
chemotherapy can have low counts, especially regarding the WBC
and Platelets. Their H&H may be low, needing a blood transfusion. It
is important for the nurse to review the lab results so that proper
medical intervention can be done.
3. Therapeutic Procedures - Educate client about treatments and
procedures. For the patient who is undergoing chemotherapy, the
nurse can educate the patient about the chemotherapy drugs,
along with side effects that they may endure while undergoing
chemo. Rationale - In order for the patient to consent to
medications/treatment/procedures, they must be educated about
each specifically and also the side effects that may occur. The
patient will be more aware of the side effects (less shocked) if they
are told prior to beginning the treatment, which can help reduce
negative feelings (especially regarding hair loss).

Assess the client’s need for palliative care/symptom management or non-

chemotherapy, palliative care is a great source to help reduce symptoms and

cancer.
