, Samenvatting Aanhoudende lage rugpijn BAS 12
Lage rugpijn → rugpijn die zich bevindt tussen de onderste ribben en bilplooien.
De rugpijn kan uitstralen naar een/beide billen en/of benen.
Fases rugpijn: acuut (0-6wkn) / subacuut (6-12 wkn) / chronisch (>12 wkn)
AANHOUDENDE LAGE RUGPIJN DIAGNOSTISCH PROCES
Indicatie fysiotherapie
• Patiënt met lage rugpijn heeft een hulpvraag die gerelateerd is aan beperkingen in ADL en/of
maatschappelijke participatie op basis van het bewegend functioneren;
• De therapeut ziet geen aanleiding tot (terug)verwijzing naar huisarts of specialist.
Rode vlaggen
Neurologisch noodgeval
• Uitval op bepaald niveau hebben → cauda-equinasyndroom.
• Moeite met plassen of ontlasting
• Toenemende motorische zwakte of uitval aanwezig
• Eventueel uitval aan twee kanten, waarbij motorische uitval dusdanig ernstig is dat de MRC-
score onder de 3 gaat zitten.
• Gevoelsstoornissen die zijn opgetreden in minder dan 48 uur.
• Wijdverspreide neurologische symptomen (bijv. Piramidale tekenen (dezelfde tekenen bij
iemand die herseninfarct heeft)) → problemen met spraak, hangen van helft van het gezicht,
problemen met slikken etc.
Fractuur
• Ernstige lage rugpijn na (hoogenergetisch) trauma.
• Ernstige lage rugpijn kan ook ontstaan na kleine trauma of zonder dat de pt zich bewust is
van een trauma → komt vaak voor bij mensen met een verhoogde kans op een fractuur.
- Hoge leeftijd
- Voorgeschiedenis van kanker
- Axiale spondylartritis
- Osteoporose
- Langdurig gebruik corticosteroïden.
Infectie
• Nachtzweten
• Rillingen en verhoogde temperatuur
• Intraveneus druggebruik
• Immunodeficiëntie (immuunsysteem werkt niet zo goed).
• Onverklaard gewichtsverlies
• Bekend zijn met voorgaande systemische infecties of risico op infectie.
• Recente chirurgische ingreep (pt kan daarna infectie oplopen).
• Huidinfectie of infecties in de urine.
Tumor
• Komt vooral voor bij jongeren < 18 jaar of ouderen > 55 jaar.
• Voorgeschiedenis van kanker.
• Onverklaard gewichtsverlies > 5kg verlies binnen een maand.
• Vermoeidheid (ernstiger dan normaal).
• Ernstige nachtelijke pijn (die niet over gaat bij wisselen van houding)
• Nachtzweren
