,Anatomie: Peritoneale caviteit en vascularisatie
Topografie
Wat horen we te weten:
• Systemen
o Tractus digestivus
o Tractus uropoeticus
o Tractus genitalis
• Bekken/-bodem
• Buikwand en peritoneum
• Vascularisatie à portale systeem en anastomosen (welke organen draineren portaal en
welke cavaal), azygos vascularisatie van de thorax en abdomen.
• Lymfedrainage
• Autonomische innervatie
Embryologie
De embryonale kiemschijf bestaat uit drie lagen:
ectoderm, mesoderm, endoderm.
Het diafragma ontstaat uit het mesoderm. Het
diafragma scheidt de toegangsholte van de buikholte
nog niet volledig. Het heet hier nog niet de
thoraxholte, omdat deze nog gevormd moet worden.
Er moet nog een aparte pericardiale caviteit,
pleuraholte en pericardioperitoneale kanalen gevormd
worden. Bij dit proces wordt de n. phrenicus
betrokken.
Het toekomstige diafragma ontwikkelt craniaal
van het hart. Door de draaiingen komt het hart
uiteindelijk boven het diafragma te liggen. Het
diafragma ontwikkelt zich dus op cervicaal
niveau. Dat is dus ook de reden dat de
nekwervels (C3, C4, C5) de spinale zenuwen
voor het diafragma voeden.

, De n. phrenicus wordt mee naar binnen getrokken en dit is niet de pure motorische zenuw
als hoe we hem kennen. De n. phrenicus loopt ook op het pericard en heeft ook afferenten
daarbij zich die in het geval van pericarditis referred pain in het phrenicus gebied kunnen
veroorzaken. Dit is een dermatoompijn (C3, C4, C5). De n. phrenicus innerveert (sensibel)
ook nog de onderzijde van het diafragma. Een hepatitis/galblaasprobleem/maagprobleem
kan het diafragma prikkelen en kan dus referred pain in de schouder geven.
à Hier zie je dat het diafragma nog niet volledig de buik en thorax scheidt.
à In week 6/7 zie je dat de scheiding volledig is door de ingroei van een soort membraan,
wat leidt tot het volledig sluiten van de thorax en het abdomen. Het linker kanaaltje sluit soms
wat lastiger en dit kan je ook in de macroscopie terugzien.
Diafragma
Bovenaanzicht:
Het diafragma bestaat uit een centrale peesplaat
(centrum tendinium) met spieren er omheen. Je
ziet ook dat het diafragma koepelvormig is. De
recessus costodiafragmaticus (loopt dan ook 360
graden rond, dus niet alleen in de midaxillair lijn) &
costomediastinalis.
Het groene puntje in het midden is de lymfe (ductus
thoracicus)

- V. hemiazygos
