6.28.2.1. When personnel report lost/stolen IDs/IPs to BDOC, they will be issued a lost/stolen ID card

flag the ID/IP in IACS, using the LEO station, with "Call LE Flag" and annotate in the flag remarks that the
card was reported lost/stolen - "confiscate if used and detain user until determination is made as to why
the lost/stolen ID/IP is being used."
6.28.2.2. Individual reporting the loss/theft must contact their First Sergeant to obtain signature. Once
the unit First Sergeant has signed the letter, the individual needs to report to the MPF for a new ID Card
or to VCC for IACS IP.
6.28.2.3. Personnel reporting stolen IDs or IACS IPs will complete an AF 1168 regarding the
circumstances of the alleged theft. A blotter entry and AF Form 3545 are conducted for only stolen ID
Cards. For lost IDs/IPs only a blotter entry with individual's information, date, time, and location of loss,
and short synopsis is required.
