Wat is interventieradiologie
Omschrijving ‘interventie radiologie’
= relatief nieuw specialisme waarbij beeldvorming gebruikt wordt om interventies
te gaan doen via minimaal invasieve procedures.
Interventieradiologie vs. chirurgie of subspecialisaties: heel vaag
Initieel was radiologie een middel om het lichaam te onderzoeken onder de
vorm van CT, RC, MR enz.

Contraststof: meestal iodine (bij gewone fluoroscopie: Barium en Jodium).
Strooistraling
= grootste stralingsbron voor de professional.
Belangrijk principe in het beschermen van een zorgverlener
o Wanneer de röntgenbuis boven de patiënt hangt is er strooistraling
richting het gezicht.
o Wanneer de röntgenbuis onder de tafel zit, wordt de strooistraling
naar de voeten gekaatst wat minder schadelijk is.
Seldinger techniek
Omschrijving ‘Seldinger Techniek’
= punctietechniek om rechtstreeks toegang te creëren tot het bloedvat. Deze
techniek is niet enkel t-van toepassing bij interventieradiologie, maar voor alle
puncties die veneus of arterieel kunnen gebeuren.
, Procedure
Stap 1: inbrengen van holle naald
Stap 2: wanneer je bloed ziet in de katheter, wordt er door de katheter
een voerdraad ingestoken.
Stap 3: voerdraad inbrengen tot 10cm.
Stap 4: eens de voerdraad erin zit, wordt de naald verwijderd.
Stap 5: eens de naald verwijderd is, dan wordt er een sheat ingestoken
Stap 6: wanneer de sheat erin zit, wordt de voerdraad verwijderd.
Punctieplaatsen
= bloedvaten die toegankelijk zijn.
Arteriën
o Radialis, femoralis, brachialis
Venen
o Jugularis, subclavia, basilica en brachialis.
Algemene complicaties en aandachtspunten
Bloedingen
= afhankelijk van de procedure.
Bloedingen zijn erger bij renale en hepatobilliaire procedures waar
weefselpuncties kunnen voorkomen.
Ook erger bij angio, neuro angio en cardiale puncties.
Verpleegkundige aandachtspunten
Letten op bloedverdunning van de patiënt
o Aspirinis: geven weinig tot geen risico op bloedingen.
o Eliques, Pradaxa, Xarelto en Arixtra: gemiddeld risico
o Marevan, Marcoumar en clopidogrel: gevaarlijk
INR controle
Aandacht voor tekenen van hypovolemie en tamponade bij cardiale
puncties.
Hemoglobine (Hb) en Hematocriet (Htc) dalingen.
Algemene complicaties
Pseudoaneurysma t.h.v. punctieplaats Ruptuur of dissectie (vooral bij
verkalking)
