Currently, chronic illnesses are the main cause of disability and preventable death
globally and the costs and prevalence linked with chronic conditions are increasing. As the
pervasiveness of chronic illnesses continues to increase, more prominence is being placed on the
development of strategies for primary care that improve the delivery of healthcare. As such,
registered nurses (RNs) play a critical role in the management of chronic illnesses and are well-
placed to improve the delivery and planning of healthcare resources in primary care. According
to Griffin (2017), the management of chronic disease comprises tasks such as obtaining the
smoking history, vital signs, encouraging exercise and wound care. Others include lifestyle
counselling and education on habits that are healthy. This paper, therefore, discusses a nurse’s
role in providing holistic care for Matthew Thornton who is living with type 2 diabetes,
hypertension and obesity. It also includes a discussion of models of care to support Matthew as
well as the national strategic framework for the patient.
