ATI Chapter 30: Suicide

Suicide = Intentional act of kiling oneself by any means
Suicide ideation = Thinking about killing oneself
Completed suicide = Suicide successfully resulting in death
Non-Suicidal Self-Injury = Self-Injury directed to the surface of the body to induce relief from a
negative feeling/cognitive state or to achieve a positive mood state.

Risk Factors: Psychiatric disorders, alcohol or substance use disorders, male gender,
increasing age, race (85% - 90% Caucasians), religion, marriage, profession, and physical
health. Military personnel/veterans, lesbian, gay, bisexual, transgender, comorbid mental illness,
Older Adults: Untreated depression, changes in functional ability, loss of loved ones.
Biological Factors: Family history, AIDS, cancers, CVD, CKD, HI, MS
Psychosocial Factors: sense of hopelessness, intense emotions, trauma
Cultural Factors: American Indian and Alaskan Native have highest rates.
Environmental Factors: Lethal methods, lack of access to mental health care
Nursing Process
1. Assessment:
- Verbal & non-verbal cues such as:
Overt statements (direct): “There is just no reason for me to go on
living.”, “My family will be better of if I’m dead”, “I wish my life was over.”,”If I kill
myself then my problems will go away.”
Covert statements (indirect): “Everything is looking pretty grim for me.”,
“The stress in my life is too much to handle.”, “I don’t feel like I can ever be happy
again”.
- Essential to ask the client if they are thinking of suicide, this will not give the client
the idea to commit suicide.
- Assess for potential suicide risk using standardized assessment tool, such as the
SAD PERSONS scale.
- Assess the client’s suicide plan
- Perform a self-assessment
- Physical Assessment Findings: Lacerations, scratches, and scars that could
indicate previous attempts at self-harm .
2. Diagnosis
- Risk for suicide
- Self-care deficit
- Sleep pattern disturbance
- Altered nutrition
- Anxiety
