TOPIC: ANGINA PECTORIS




Introduction

 Angina: mean sense of suffocation or pain.
 Pectoris: means chest.




 Therefore angina pectoris: refers to as clinical syndrome usually characterizes by:
 Episode of pain
 Proximal (random or sudden outburst) pain
 Radiating pain
 Excruciating (intense, extreme or great agonizing or anguish) pain
 Chocking pain
 Pressure in the anterior chest
 Suffocation etc all cause by lack of oxygen to the heart muscle fibres resulting to
physical exertion (expenditure of physical or mental effort) or emotional stress which
also increase by work load of the heart
 The pain usually last for short period of time and subside when precipitating factor is off.




Types of Angina pectoris

1) The stable (typical angina or classical angina).
2) chronic stable
3) Unstable (atypical or crescendo or pre-infective or progressive angina)
4) Variant or prinzmetal’s angina or vasospastic angina.
5) Nocturnal Angina.
6) Angina decubitus
1. Stable Angina (typical angina or classical angina):
 Is a sudden chest pain which is retrosternal (behind the sternum) in nature.
 It occurs intermittently during exercise or stress and may radiate to the left shoulder, left
arm, and occasionally to the neck or jaw.
 The pattern of onset, duration, and intensity, remain the same i.e. does not increase in
severity.
 It lasts few minute for about 2-3 minutes.
 It is usually relief by controlling the activity that cause the pain or medication or relieve by
rest.
2. Chronic stable angina: Is a type of angina which is the same with stable angina but duration
of the pain last for a long period of time.
3. Unstable angina (Atypical, or crescendo or pre-infective or progressive angina) : Is the type
of angina in which the pain is like that of stable angina in nature and last for 12-24 hours
and some time is severe in nature or progressively increase in frequency, intensity and
duration and may lead to myocardial infarction.
4. Variant angina (vasospastic angina or prinzmental’s angina)
 This is the type of angina in which the pain occurs every day even at rest usually between
midnight and early morning also not precipitate by exertion.
 The pain also associated with coronary artery spasm and it has a high risk of infarction,
accompanied by transient ST-elevation in the ECG
5. Nocturnal (nightly) Angina:
 Is the type of angina that occur due to ventricular failure
 Pain occurs only at night usually during sleeping.
