History of Present Illness (HPI):
Onset: The patient describes that headaches have been present for several years but progressed in

photophobia, phonophobia, and sometimes dizziness are associated with the complaints. No vomiting.

stress and not sleeping well for the last few weeks. No trauma, no recent change in his vision, or
neurological deficits.
